垂体瘤术中发生低体温患者危险因素及术后并发症分析

Analysis of Risk Factors in Patients with Hypothermia during Pituitary Adenoma Surgery and Postoperative Complications

目的探究垂体瘤患者术中发生低体温的危险因素及术后并发症发生率。方法本研究纳入2022年11月至2023年6月于福建医科大学附属第一医院滨海院区神经外科接受手术治疗的70例垂体瘤患者为研究对象。手术方式均采用神经内镜经鼻-蝶入路术式。利用一次性食道体温传感器监测患者术中体温情况。分析患者术后并发症发生情况,应用多因素Logistic回归分析发生低体温的危险因素。结果低体温组术后电解质紊乱、新发垂体功能低下、尿崩的发生率高于正常体温组(P<0.05),低体温组平均住院时长大于正常体温组(P<0.05)。Logistic回归分析显示发生低体温的危险因素是术前甲状腺激素低下、低BMI。结论垂体瘤术中低体温发生率高,低体温患者并发症发生率高,低BMI、术前甲状腺激素水平低下的患者发生低体温的风险较大。

Objective To investigate the risk factors for intraoperative hypothermia and the incidence of postoperative complications in pituitary tumor patients.Methods Seventy patients with pituitary tumors who underwent surgical treatment at the Department of Neurosurgery,Binhai Hospital,The First Affiliated Hospital of Fujian Medical University from November 2022 to June 2023 were included in this study.Neuroendoscopic transnasal-pterygoid approach was used for all patients.Disposable esophageal temperature sensors were utilized to monitor the patients'intraoperative temperature.The occurrence of postoperative complications in patients was analyzed,and multivariate logistic regression was applied to analyze the risk factors for the occurrence of hypothermia.Results The incidence of postoperative electrolyte disorders,new-onset hypopituitarism,and dysuria was higher in the hypothermia group than in the normothermia group(P<0.05),and the average length of hospital stay in the hypothermia group was longer than that in the normothermia group(P<0.05).Logistic regression analysis showed that the risk factors for the occurrence of hypothermia were preoperative low thyroid hormone,and low BMI.Conclusions There was a high incidence of intraoperative hypothermia in pituitary tumors,and a high rate of hypothermic patients had a high complication rate,and patients with low BMI and low preoperative thyroid hormone levels were at greater risk of developing hypothermia.
