糖皮质激素、血管扩张剂联合高压氧治疗对突发性耳聋伴和不伴糖尿病患者的短期临床疗效对比

Comparison of Short-Term Clinical Efficacy of Glucocorticoid,Vasodilator and Hyperbaric Oxygen in the Treatment of Sudden Deafness with and without Diabetes

目的观察糖皮质激素、血管扩张剂联合高压氧治疗对突发性耳聋伴和不伴糖尿病患者的短期临床疗效对比。方法选取2020年1月至2023年6月在南京市溧水区人民医院明确诊断为突发性耳聋的患者61例,其中不合并糖尿病的患者36例设为对照组,合并糖尿病的患者25例设为观察组,对比分析两组间治疗前后500 Hz、1000 Hz、2000 Hz的纯音听阈水平、治疗总有效率及平均住院时间。结果两组患者治疗前的一般资料包括年龄、性别、发病时间、纯音听阈水平比较无统计学差异(P>0.05)。治疗后1周,两组患者的听阈水平均较治疗前改善(P<0.05),而对照组的纯音听阈水平优于观察组,治疗总有效率要高于观察组(P<0.05)。两组患者平均住院时间比较无统计学差异(P>0.05)。结论糖皮质激素、血管扩张剂及高压氧治疗对突发性耳聋患者伴与不伴糖尿病患者均有显著疗效,可使患者获得良好听力恢复,且安全可靠。然而,合并糖尿病患者较不合并糖尿病患者对治疗方案预后稍差,在治疗过程中应严格管理血糖控制水平。

Objective To observe the short-term clinical efficacy of glucocorticoid,vasodilator and hyperbaric oxygen therapies in the treatment of sudden deafness with and without diabetes.Methods From January 2020 to June 2023,61 patients with sudden deafness who were diagnosed in Nanjing Lishui People's Hospital were selected.Among them,36 patients without diabetes were set as the control group,and 25 patients with diabetes were set as the observation group.The pure tone hearing threshold levels of 500 Hz,1000 Hz,2000 Hz before and after treatment,the total effective rate of treatment,and the average length of stay between the two groups were compared and analyzed.Results There was no statistically significant difference in the clinical data before treatment between the two groups of patients,including age,gender,onset time,and pure tone hearing threshold level(P>0.05).One week after treatment,the hearing threshold levels of both groups of patients were improved compared to before treatment(P<0.05).However,the pure tone hearing threshold level of the control group was better than that of the observation group,and the total effective rate of treatment was higher than that of the observation group(P<0.05).There was no statistically significant difference in the average hospitalization time between the two groups of patients(P>0.05).Conclusions Glucocorticoid,vasodilator and hyperbaric oxygen therapies have significant effects on sudden deafness patients with and without diabetes,which can obtain good hearing recovery,and are safe and effective.However,patients with diabetes have poor prognosis,communication with patients during the treatment process should be emphasized,and strict management of blood sugar control should be highlighted.
