经尿道膀胱肿瘤电切术患者采取快速康复外科理念干预的价值

Value of enhanced recovery after surgery in patients undergoing transurethral resection of bladder tumors

目的 探讨快速康复外科理念应用在经尿道膀胱肿瘤电切术治疗中的效果。方法 选择2020年6月至2022年6月收治的经尿道膀胱肿瘤电切术患者78例,依据随机数字表法分为对照组和观察组,每组39例。对照组执行传统护理,观察组执行快速康复外科理念干预。比较2组干预前、后舒适度、疼痛评分、临床各指标水平及并发症发生情况。结果 观察组临床各指标包括手术时间、术后排气时间、住院时间低于对照组(P<0.05);干预前2组舒适度、疼痛评分差异无统计学意义(P>0.05),干预后观察组舒适度评分高于对照组、疼痛评分低于对照组(P<0.05);2组护理后,平均尿流率及血清前列腺特异抗原(PSA)水平较干预前显著改善,且观察组改善程度更优(P<0.05)。观察组并发症发生率为5.13%低于对照组的20.51%,差异有统计学意义(P<0.05)。结论 经尿道膀胱肿瘤电切术患者的护理服务中融入快速康复外科理念效果明显,可提升患者自身舒适度,并减轻自身疼痛感,同时改善患者排尿状况,缩短患者住院时间,有效规避了并发症发生,临床上可借鉴及推广。

Objective To explore the effect of enhanced recovery after surgery(ERAS)in patients undergoing transurethral resection of bladder tumors.Methods A total of 78 bladder tumor patients treated with transurethral resection in the hospital from June 2020 to June 2022 were selected.They were randomly divided into observation group and control group,with 39 cases in each group.Patients in the control group received traditional nursing,and those in the observation group were managed by ERAS-based intervention.Comfort level,pain score,clinical indicators and complication rate of the two groups were compared before and after intervention.Results The clinical indexes,including operation time,postoperative exhaust time and length of stay were significantly shorter in the observation group than those of the control group(P<0.05).There were no significant differences in comfort and pain scores between the two groups before intervention(P>0.05).After intervention,the comfort score of the observation group was significantly higher than that of the control group,and the pain score was significantly lower than that of the control group(P<0.05).After the intervention,the mean urine flow rate and serum-prostate specific antigen(PSA)level of the two groups were significantly improved compared with that before intervention,which were more pronounced in the observation group(P<0.05).The complication rate was significantly lower in the observation group than that of control group(5.13%vs 20.51%,P<0.05).Conclusion ERAS-based nursing has a significant effect on patients undergoing transurethral resection of bladder tumors,which can improve the comfort and urination,alleviate pain,shorten the length of stay and prevent the occurrence of complications.It is worthy of clinical reference and promotion.
