关节镜下改良双排缝合与单排缝合技术治疗中型肩袖撕裂的疗效比较

Comparison of arthroscopic modified double-row suture and single-row suture technique in the repair of medium-sized rotator cuff tear

目的比较关节镜下改良双排缝合与单排缝合技术治疗中型肩袖撕裂的临床疗效。方法将56例中型肩袖撕裂患者根据随机数字表法分为观察组(采用改良双排缝合技术治疗)和对照组(采用单排缝合技术治疗),各28例。比较两组手术情况、疼痛VAS评分、UCLA评分、ASES评分、Constant评分及肩关节活动度。结果患者均获得随访,时间6~10(7.58±2.46)个月。手术时间、术中出血量两组比较差异无统计学意义(P>0.05)。术后1、3、6个月,疼痛VAS评分观察组均低于对照组(P<0.05);两组UCLA评分、ASES评分、Constant评分比较差异均无统计学意义(P>0.05)。术后6个月肩关节外展、前屈、外旋活动度两组均较术前改善(P<0.05),两组比较差异均无统计学意义(P>0.05)。结论采用关节镜下改良双排缝合技术与单排缝合技术治疗中型肩袖撕裂均能有效提高肩关节功能,但改良双排缝合技术在术后疼痛改善方面更优。

Objective To compare the clinical efficacy of modified double-row suture and single-row suture technique in the treatment of medium-sized rotator cuff tear under arthroscope.Methods The 56 patients with medium-sized rotator cuff tears were divided into observation group(using modified double-row suture technique)and control group(using single-row suture technique)according to random number table method,with 28 cases in each group.The surgical conditions,pain VAS,UCLA score,ASES score,Constant score and shoulder motion were compared between the two groups.Results All patients were followed up for 6~10(7.58±2.46)months.There were no statistical differences for the operation time and intraoperative blood loss between two groups(P>0.05).At 1,3 and 6 months after surgery,the pain VAS of observation group was lower than that of control group(P<0.05).There were no statistical significance for UCLA scores,ASES scores and Constant scores between the two groups(P>0.05).Six months after surgery,the abductor,anterior flexion and external rotation of shoulder joint in the two groups were improved,compared with those before surgery(P<0.05),and there were no statistical significance between the two groups(P>0.05).Conclusions In the treatment of medium-sized rotator cuff tear,both modified double-row suture technique and single-row suture technique under arthroscope can effectively improve shoulder joint function,but modified double-row suture technique is better in postoperative pain improvement.
