湿润烧伤膏治疗糖尿病患者包皮环切术后创面疗效分析

Clinical Efficacy of MEBO in the Treatment of Postoperative Wounds of Circumcision in Diabetic Patients

目的分析探讨湿润烧伤膏治疗糖尿病患者包皮环切术后创面的临床疗效。方法选取2020年9月至2022年9月天津市北辰医院收治的156例行包皮环切术治疗的合并糖尿病的包茎或包皮过长患者作为研究对象,并按照不同治疗方法将其分为研究组(78例)与对照组(78例),研究组患者包皮环切术后创面采用湿润烧伤膏治疗,对照组患者包皮环切术后创面采用凡士林油纱治疗,对比观察两组患者创面疼痛程度、创面水肿情况、完全脱钉时间、创面愈合时间以及感染、血肿、切口裂开等并发症发生情况与患者满意度。结果研究组患者术后第2、3、5天创面疼痛评分及水肿评分均明显低于对照组(创面疼痛评分:t=6.539、6.404、13.840,P均<0.001;创面水肿评分:t=5.192、5.255、10.980,P均<0.001),完全脱钉时间及创面愈合时间均明显短于对照组(t=7.736、6.718,P均<0.001),并发症发生率明显低于对照组(χ^(2)=10.078,P=0.002),对治疗效果的满意度明显优于对照组(Z=-4.403,P<0.001)。结论湿润烧伤膏治疗糖尿病患者包皮环切术后创面,可有效降低术后创面疼痛及水肿程度,加快创面愈合,减少感染、血肿、切口裂开等并发症发生风险,提高患者满意度。

Objective To analyze the clinical efficacy of MEBO in the treatment of postoperative wounds of circumcision in diabetic patients.Methods 156 diabetic patients with phimosis or redundant prepuce,admitted to Tianjin Beichen Hospital for circumcision from September 2020 to September 2022,and divided into study group(n=78)and control group(n=78)based on the different treatments they received.The postoperative wounds of patients in the study group were treated with MEBO,while the postoperative wounds of patients with Vaseline gauze in the control group.The degree of wound pain,wound edema condition,complete nail removal time,wound healing time and occurrence of compli⁃cations including infection,hematoma,incision dehiscence as well as patients’satisfaction were compared between the two groups.Results Respectively on day 2,3 and 5 after circumcision,the wound pain scores and edema scores of patients in the study group were all significantly lower than that in the control group(wound pain score:t=6.539,6.404 and 13.840,all P<0.001;wound edema score:t=5.192,5.255 and 10.980,all P<0.001).The complete nail removal time and wound healing time were both obviously shorter in the study group compared with the control group(t=7.736 and 6.718,both P<0.001),and the incidence of complications was markedly lower in the study group(χ^(2)=10.078,P=0.002).The satisfaction of patients towards treatment effect was obviously superior to that in the control group(Z=-4.403,P<0.001).Conclusion In the treatment of postoperative wounds of circumcision in diabetic patients MEBO can effectively relieve the postoperative wound pain and edema degree of patients,speed up the wound healing,and decrease the risk of complications including infection,hematoma and dehiscence of incision,thus realizing a much high degree of patients’satisfaction.
