2015—2021年大理州卫生人力资源配置公平性分析

Fairness Analysis of Human Resource Allocation in Medical Health Industry in Dali Prefecture from 2015 to 2021

目的:探讨2015—2021年云南省大理州卫生人力资源配置的公平性,为区域卫生人力资源的优化配置提供依据。方法:对大理州卫生人力资源量进行统计描述,采用基尼系数和泰尔指数从人口、地理配置两方面对卫生人力资源配置的公平性进行分析评价。结果:2015—2021年大理州卫生技术人员、执业(助理)医师、注册护士按人口分布的基尼系数分别在0.248~0.287、0.257~0.289、0.288~0.337之间,按地理面积配置的基尼系数分别在0.483~0.527、0.487~0.526、0.519~0.579之间;按人口配置的总泰尔指数在0.101~0.142、0.120~0.153、0.138~0.209之间,按地理面积配置的总泰尔指数分别在0.420~0.504、0.426~0.488、0.489~0.649之间。结论:大理州卫生人力资源按人口配置的公平性优于按地理面积配置的公平性,卫生技术人员配置的公平性优于执业(助理)医师,而注册护士的公平性最差,区域间差异是影响卫生人力资源配置公平性的主要因素。

OBJECTIVE To explore the fairness of the allocation of health human resources in Dali prefecture from 2015 to2021,and to provide a basis for the optimal allocation of regional health human resources.METHODS The amount of health human resources in Dali prefecture was statistically described,and the Gini coefficient and Thiel index methods were used to analyze and evaluate the fairness of health human resources allocation from the aspects of population and geographical allocation.RESULTS The Gini coefficients of health technicians,practicing(assistant) physicians and registered nurses in Dali prefecture were between 0.248~0.287,0.257~0.289 and 0.288~0.337 according to population distribution in 2015~2021,while the Gini coefficients of Dali health technicians,practicing(assistant) physicians and registered nurses were between 0.248~0.287,0.257~0.289 and 0.288~0.337,respectively.The total Thiel index of health technicians,practicing(assistant) physicians and registered nurses in Dali was between 0.101~0.142,0.120~0.153,0.138~0.209,and the total Thiel index by geographical area was between 0.420~0.504,0.426~0.488,0.489~0.649 from 2015 to 2021.CONCLUSION The fairness of health human resources allocation by population is better than that of geographical area in Dali,the fairness of health technician allocation is better than that of practicing(assistant) physicians,and the fairness of registered nurses is the worst,and regional differences are the main factors affecting the fairness of health human resources allocation.
