上海市三级甲等综合医院腹膜透析中心家庭访视工作现状调查

A Cross-sectional Survey on Home Visits in Peritoneal Dialysis Centers of Tertiary General Hospitals

目的:调查上海市三级甲等综合医院腹膜透析中心家庭访视工作现状及存在的问题,为相关工作的顺利开展提供参考。方法:2021年9月,采用自行设计的“腹膜透析中心家庭访视调查问卷”,对上海市19所三级甲等综合医院腹膜透析中心进行问卷调查。结果:有17所医院开展了患者家庭访视工作,14所医院已制定了腹膜透析家庭访视相关制度,年均家庭访视人次数仅为13人次。参与访视的医护人员以腹膜透析专职护士为主,其次为腹膜透析专职医生。家庭访视内容主要为观察患者居家腹膜透析操作环境、操作规范性、并发症处理、家属支持情况、容量管理、饮食指导等。18所医院认为开展家访有困难,8所医院认为家访后续监测存在困难。结论:上海市大部分三级甲等综合医院腹膜透析中心开展了家庭访视工作,但家庭访视人员以腹膜透析专职护士为主,家庭访视工作的开展受人力资源与结构、患者居住地、经济费用等因素的影响。

OBJECTIVE To investigate the development of family visits in peritoneal dialysis center of Grade 3A General Hospital,understand the status and find new forms of home visits for reference.METHODS In September of 2021,by a self-designed"PD center home visit questionnaire",19 peritoneal dialysis centers in Shanghai were investigated with a self-designed questionnaire on family visits of peritoneal dialysis centers.RESULTS Among 19 hospitals,17 peritoneal dialysis centers carried out home visits,and 14 peritoneal dialysis centers had formulated related family visits,with only 13 family visits per year.The main participants were full-time dialysis nurses,followed by full-time dialysis doctors,dietitian,and social workers.The main contents of home visits were observation of patients'home environment,followed by peritoneal dialysis related operations,complications management,family support,capacity management,dietary guidance,etc.18 peritoneal dialysis centers found it difficult to carry out home visits,and 8 peritoneal dialysis centers found it difficult to conduct follow-up monitoring of home visits.CONCLUSION Most of the peritoneal dialysis centers of grade A general hospitals in Shanghai have carried out home visits,but professional nurses mainly perform peritoneal dialysis.The development of home visits is affected by human resources and structure,patient residence,economic costs and other factors.
