血清Ghrelin、Omentin-1及IGF-1在特发性矮小症患儿中的表达变化及其与生长发育的相关性分析 被引量:2

摘要 目的分析血清生长激素释放激素(Ghrelin)、网膜素1(Omentin-1)及胰岛素样生长因子1(IGF-1)在特发性矮小症患儿中的表达变化及其与生长发育的相关性。方法回顾性分析2019年10月至2022年10月无锡市第九人民医院收治的特发性矮小症患儿100例作为观察组,再选择同期体检健康儿童80名作为对照组。比较两组血清Ghrelin、Omentin-1及IGF-1水平差异。采用Pearson检验分析血清Ghrelin、Omentin-1、IGF-1水平与特发性矮小症生长发育的相关性。采用多因素Logistic回归分析影响特发性矮小症患儿生长发育的独立危险因素。采用受试者工作特征曲线(ROC)分析血清Ghrelin、Omentin-1、IGF-1水平预测特发性矮小症患儿生长发育的价值。结果观察组血清Ghrelin、Omentin-1、IGF-1水平分别为(15.42±3.21)、(49.56±4.21)、(156.32±8.94)ng/mL,低于对照组[(25.65±6.54)、(62.31±6.85)、(254.63±6.52)ng/mL],差异均有统计学意义(P<0.05)。Pearson相关性检验显示,血清Ghrelin、Omentin-1和IGF-1水平与特发性矮小症体重指数呈正相关(r=0.769、0.833、0.774,P<0.05)。多因素Logistic回归分析结果显示,血清Ghrelin、Omentin-1及IGF-1水平是特发性矮小症患儿生长发育的独立危险因素(OR=4.491,95%CI:1.778~11.348;OR=4.604,95%CI:1.613~13.139;OR=4.821,95%CI:1.628~14.280;P<0.05)。ROC曲线分析结果显示,血清Ghrelin、Omentin-1及IGF-1水平预测特发性矮小症患儿发生曲线下面积(AUC)分别为0.920、0.942、0.765,均具有极高预测价值(P<0.05)。结论随着特发性矮小症的发生,血清Ghrelin、Omentin-1及IGF-1水平降低,该3项指标在预测特发性矮小症生长发育中具有极高的价值,可用于评估特发性矮小症患儿生长发育。 Objective To analyze the expression changes of serum growth hormone releasing hormone(Ghrelin),omentin-1(Omentin-1),and insulin-like growth factor 1(IGF-1)in children with idiopathic short stature and their correlation with growth and development.Methods A retrospective analysis was conducted on 100 children with idiopathic short stature admitted to Wuxi Ninth People's Hospital from October 2019 to October 2022 as the observation group,and 80 healthy children who underwent physical examination during the same period were selected as the control group.The differences in levels of serum Ghrelin,Omentin-1,and IGF-1 between the two groups were compared.The correlation between serum levels of Ghrelin,Omentin-1,and IGF-1 and the growth and development of idiopathic dwarfism was analyzed by Pearson's test.The independent risk factors affecting the growth and development of children with idiopathic dwarfism were analyzed using multivariate Logistic regression.The value of serum Ghrelin,Omentin-1,and IGF-1 levels in predicting the growth and development of children with idiopathic dwarfism was analyzed using the receiver operating characteristic(ROC)curve.Results The serum levels of Ghrelin,Omentin-1,and IGF-1 in the observation group were(15.42±3.21),(49.56±4.21),(156.32±8.94)ng/mL,respectively,which were lower than those in the control group[(25.65±6.54),(62.31±6.85),and(254.63±6.52)ng/mL],and the differences were statistically significant(P<0.05).Pearson correlation test showed that serum levels of Ghrelin,Omentin-1,and IGF-1 were positively correlated with the body mass index of idiopathic dwarfism(r=0.769,0.833,0.774;P<0.05).The results of multivariate logistic regression analysis showed that serum levels of Ghrelin,Omentin-1,and IGF-1 were independent risk factors for growth and development in children with idiopathic dwarfism(OR=4.491,95%CI:1.778-11.348;OR=4.604,95%CI:1.613-13.139;OR=4.821,95%CI:1.628-14.280;P<0.05).The ROC curve analysis results showed that serum Ghrelin,Omentin-1,and IGF-1 levels had extremely high predictive value(P<0.05)in predicting the area under the curve(AUC)of children with idiopathic dwarfism,with values of 0.920,0.942 and 0.765,respectively.Conclusion With the occurrence of idiopathic dwarfism,serum levels of Ghrelin,Omentin-1,and IGF-1 decrease.These three indicators have high value in predicting the growth and development of idiopathic dwarfism,and can be used to evaluate the growth and development of children with idiopathic dwarfism.
