空腹胰岛素、C肽、尿微量白蛋白结合hs-CRP在2型糖尿病肾病诊断中的临床意义

Clinical Significance of Fasting Insulin,C-Peptide,and Urinary Microalbumin Binding hs-CRP in the Diagnosis of Type 2 Diabetic Nephropathy

目的 分析空腹胰岛素、C肽、尿微量白蛋白与超敏C反应蛋白(hypersensitive C-reactive protein, hsCRP)联合检测对2型糖尿病肾病的诊断价值。方法 回顾性选取2021年1月-2022年12月宁德师范学院附属宁德市医院收治的2型糖尿病肾病患者78例作为观察组;同期健康体检者78例为对照组。对比两组空腹胰岛素、C肽、尿微量白蛋白和hs-CRP水平,并对不同程度患者各参数予以对比;经受试者工作特征曲线分析指标联合评估价值。结果 观察组空腹胰岛素、C肽、尿微量白蛋白、hs-CRP水平均高于对照组,差异有统计学意义(P<0.05)。ROC曲线图显示,空腹胰岛素、C肽、尿微量白蛋白及hs-CRP 4项指标联合检测的诊断效能最佳,灵敏度和特异度分别为97.85%和93.75%。结论 2型糖尿病肾病患者多项血清指标异常,联合检测可对疾病予以准确诊断。

Objective To analyze the diagnostic value of fasting insulin,C-peptide,urinary microalbumin and hyper-sensitive C-reactive protein(hs-CRP)in type 2 diabetic nephropathy.Methods 78 patients with type 2 diabetic ne-phropathy admitted to Ningde Municipal Hospital,Affiliated to Ningde Normal University from January 2021 to De-cember 2022 were selected as the observation group.In the same period,78 healthy subjects were the control group.The levels of fasting insulin,C-peptide,urinary microalbumin and hs-CRP were compared between the two groups,and the parameters of patients with different degrees were compared.The value was evaluated jointly by the analysis index of the subject's working characteristic curve.Results Fasting insulin,C-peptide,urinary microalbumin and hs-CRP levels in the observation group were higher than those in the control group,the difference was statistically signifi-cant(P<0.05).The ROC curve graph showed that the diagnostic efficacy of the four indicators of fasting insulin,C-peptide,urinary microalbumin and hs-CRP combined test was the best,with a sensitivity and specificity of 97.85%and 93.75%,respectively.Conclusion Type 2 diabetic nephropathy patients have many abnormal serum indexes,and the combined test can accurately diagnose the disease.
