医院-社区-家庭个体化签约服务对老年T2DM患者服药依从性的影响

Effect of Hospital-community-family Individualized Contracting Service on Medication Compliance in Elderly Patients with T2DM

目的 探究医院-社区-家庭个体化签约服务对老年2型糖尿病(type 2 diabetes mellitus, T2DM)患者服药依从性的影响。方法 选取2020年1月-2021年12月厦门市翔安区新店卫生院收治的112例老年T2DM患者作为研究对象,采用简单随机法将其分为对照组(n=56)以及观察组(n=56)。对照组给予常规护理,观察组在对照组的基础上给予医院-社区-家庭个体化签约服务进行干预。比较干预前、干预6个月后两组患者服药依从性[采用中文版服药信念特异性问卷(Beliefs about Medical Questionnaire-Specific, BMQ-Specific)评估]、糖尿病知识掌握情况[采用糖尿病知识问卷(Audit of Diabetes Knowledge, AD-Knowl)评估]以及血糖水平差异。结果 干预后,两组患者BMQ-Specific、AD-Knowl评分均高于干预前,且观察组评分高于对照组,差异有统计学意义(P<0.05);干预后,两组患者血糖指标均低于干预前,且观察组指标低于对照组,差异有统计学意义(P<0.05)。结论 采用医院-社区-家庭个体化签约服务对老年T2DM患者进行干预后,有效提高患者服药依从性以及疾病知识掌握程度,有效改善血糖指标。

Objective To explore the effect of hospital-community-family individualized contracting services on medi-cation compliance in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of one hundred and twelve elderly T2DM patients admitted to Xindian Health Center of Xiangan District from January 2020 to December 2021 were selected as the study objects and divided into control group(n=56)and observation group(n=56)by simple randomization method.The control group was given routine nursing,and the observation group was given hospital-community-family individualized contracting services on the basis of the control group for intervention.The differ-ences of medication compliance[evaluated using Beliefs About Medical Questionnaire-specific(BMQ-Specific)],dia-betes knowledge[evaluated using Audit Of Diabetes Knowledge(AD-Knowl)]and blood glucose level were compared between the two groups before intervention and 6 months after intervention.Results After intervention,BMQ-Specific and AD-Knowl scores in both groups were higher than before intervention,and the above scores in observation group were higher than those in control group,the difference was statistically significant(P<0.05).After infervention,blood glucose indexes in both groups were lower than before intervention,and the above indexes in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion After the intervention of the hospital-community-family individualized contracting service for elderly T2DM patients,the medi-cation compliance and disease knowledge of the patients were effectively improved,and the blood glucose index was effectively improved.
