三酰甘油-葡萄糖指数与老年2型糖尿病左心室收缩功能改变的关系

Relationship between Triacylglycerol-glucose Index and Left Ventricular Systolic Function Changes in Elderly Patients with Type 2 Diabetes

目的探讨三酰甘油-葡萄糖指数(triglyceride-glucose,TyG)与老年2型糖尿病(Type 2 diabetes mellitus,T2DM)左心室收缩功能改变的关系。方法选择2020年1月—2021年1月于厦门大学附属第一医院杏林分院收治的老年T2DM患者104例为研究对象,根据TyG指数四分位间距分为Q1(5.52≤TyG<8.56,n=26)、Q2(8.56≤TyG<8.71,n=26)、Q3(8.71≤TyG<9.32,n=26)、Q4(9.32≤TyG<10.55,n=26)组,各组均予以实验室指标、超声检查,对比各组检测结果,分析各指标与TyG指数的相关性。结果4组患者年龄、性别比较,差异无统计学意义(P>0.05),4组糖尿病病程、BMI比较,差异有统计学意义(P<0.05),4组患者餐后2 h血糖、糖化血红蛋白、总胆固醇比较,差异有统计学意义(P<0.05)。4组患者舒张末期左心室内径、收缩末期左心室内径、舒张末期室间隔厚度、舒张末期左心室后壁厚度和左心室射血分数比较,差异有统计学意义(P<0.05)。Pearson相关分析显示,TyG指数与糖尿病病程、体质量、糖化血红蛋白、总胆固醇、舒张末期左心室内径、收缩末期左心室内径、舒张末期室间隔厚度、舒张末期左心室后壁厚度呈正相关,与左心室射血分数呈负相关(P<0.05)。结论三酰甘油-葡萄糖指数与老年T2DM左心室收缩功能改变有一定的联系。

Objective To explore the relationship between triglyceride-Glucose(TyG)index and changes in left ventricular systolic function in elderly patients with type 2 diabetes(T2DM).Methods A total of 104 elderly patients with(T2DM)admitted to Xinglin Branch of the First Affiliated Hospital of Xiamen University from January 2020 to January 2021 were selected as the study objects.According to the interquartile interval of TyG index,they were divided into Q1(5.52≤TyG<8.56,n=26),Q2(8.56≤TYG<8.71,n=26),Q3(8.71≤TyG<9.32,n=26)and Q4(9.32≤TyG<10.55,n=26)groups were subjected to laboratory indicators and ultrasonic examinations.The detection results of each group were compared to analyze the correlation between each indicator and TyG index.Results There were no statistically significant differences in age and gender among the 4 groups(P>0.05);there were statistically significant differences in diabetes course and BMI among the 4 groups(P<0.05);there were statistically significant differences in total cholesterol,glycosylated hemoglobin and 2-hour postprandial blood glucose among the 4 groups(P<0.05).There were statistically significant differences in end-diastolic left ventricular diameter,end-systolic left ventricular diameter,end-diastolic ventricular septal thickness,end-diastolic left ventricular posterior wall thickness and left ventricular ejection fraction among the 4 groups(P<0.05).Pearson correlation analysis showed that TyG index was positively correlated with diabetes course,body mass,glycated hemoglobin,total cholesterol,end-diastolic left ventricular diameter,end-systolic left ventricular diameter,end-diastolic septal thickness and end-diastolic left ventricular posterior wall thickness,and negatively correlated with left ventricular ejection fraction(P<0.05).Conclusion There is a certain relationship between triacylglycerol-glucose index and changes in left ventricular systolic function in elderly patients with(T2DM).
