尿糖与尿微量白蛋白联合检验糖尿病早期肾损伤的价值

The Value of Combined Urine Glucose and Urine Microalbumin in Detecting Early Renal Damage in Diabetes

目的探讨采用尿糖、尿微量白蛋白联合检验方式对糖尿病早期肾损伤进行诊断的临床价值。方法选取2021年1月—2022年1月期间在牡丹人民医院接受治疗的糖尿病早期肾损伤的100例患者为研究对象,设为观察组,另外选择同期到本院接受常规检查的体检者100例作为对照组,比较两组尿糖、尿微量白蛋白水平,尿糖与尿微量白蛋白单独以及联合检验诊断结果,分析尿糖与尿微量白蛋白单独以及联合检验诊断效能。结果观察组尿糖、尿微量白蛋白水平分别为(3.05±1.84)mmol/L、(38.51±8.82)mg/L,对照组尿糖、尿微量白蛋白水平分别为(1.29±0.35)mmol/L、(16.61±3.21)mg/L,差异有统计学意义(P<0.05)。联合检验特异度为90.00%、灵敏度为98.00%、准确率为94.00%,联合检验和临床综合诊断一致性良好,Kappa值为0.880。结论糖尿病早期肾损伤发生后,患者尿糖、尿微量白蛋白含量显著增长。可以将尿糖、尿微量白蛋白联合检验作为糖尿病早期肾损伤的诊断方法。

Objective To explore the clinical value of using urine glucose and urine microalbumin combined testing method to diagnose early renal damage in diabetes.Methods A total of 100 patients with early diabetic kidney injury who received treatment in Mudan People's Hospital from January 2021 to January 2022 were selected as the study objects and set as the observation group,and 100 physical examination subjects who received routine examination in Mudan People's Hospital during the same period were selected as the control group.The diagnostic results of urine glucose,urinary microalbumin levels,urinary glucose and urinary microalbumin levels alone and combined were compared between the two groups,and the diagnostic efficacy of urine glucose and urinary microalbumin levels alone and combined were analyzed.Results The levels of urinary glucose and urinary microalbumin in the observation group were(3.05±1.84)mmol/L and(38.51±8.82)mg/L,respectively,while those in the control group were(1.29±0.35)mmol/L and(16.61±3.21)mg/L,respectively,the difference was statistically significant(P<0.05).The specificity,sensitivity and accuracy of the combined test were 90.00%,98.00%and 94.00%,and there was good consistency between the combined test and clinical comprehensive diagnosis,with Kappa value of 0.880.Conclusion After the occurrence of kidney injury in the early stage of diabetes,the content of urinary sugar and urinary microalbumin increased significantly.The combined test of urinary sugar and urinary microalbumin can be used as a diagnostic method for early renal injury in diabetes mellitus.
