二次妊娠孕妇先兆子痫再发的相关因素

Factors Related to the Recurrence of Preeclampsia in SecondTrimester Pregnant Women

目的探讨二次妊娠孕妇先兆子痫再发的相关因素。方法回顾性分析2019年3月至2021年5月驻马店市中心医院接收的148例有先兆子痫病史的二次妊娠孕妇的临床资料,将再次发生先兆子痫的40例孕妇纳入发生组,未发生先兆子痫的108例正常孕妇纳入未发生组,比较两组患者一般资料并对二次妊娠孕妇先兆子痫再发的相关因素进行探析。结果本研究中有先兆子痫病史的148例二次妊娠孕妇中40例(27.03%)发生先兆子痫。发生组与未发生组孕妇初次发病年龄、妊娠间隔时间、孕前体重指数(BMI)、先兆子痫家族史比较,差异有统计学意义(P<0.05),两组初次发病孕周、终止妊娠孕周、收缩压、舒张压、糖尿病病史比较,差异无统计学意义(P>0.05);多项logistic回归分析结果显示,初次发病年龄≥35岁、妊娠间隔时间≥5 a、孕前肥胖或超重、有先兆子痫家族史是二次妊娠孕妇再发生先兆子痫的相关因素(OR>1,P<0.05)。结论初次发病年龄≥35岁、妊娠间隔时间≥5 a、孕前肥胖或超重、有先兆子痫家族史是二次妊娠孕妇再发生先兆子痫的相关因素。

Objective To explore the related factors of recurrent preeclampsia in second trimester pregnant women.Methods A retrospective analysis was conducted of the clinical data of 148 pregnant women with a history of preeclampsia who underwent secondary pregnancy from March 2019 to May 2021 at Zhumadian Central Hospital.A total of 40 pregnant women with recurrent preeclampsia were included in the occurrence group,and 108 normal pregnant women without preeclampsia were included in the non occurrence group.The general information of the two groups of patients were compared and the related factors of the recurrence of preeclampsia in second trimester pregnant women was explored.Results In this study,40(27.03%)of 148 cases secondary pregnant women with a history of preeclampsia developed preeclampsia.There were differences between the occurrence group and the non occurrence group in terms of initial onset age,gestational interval,pre pregnancy body mass index(BMI),and family history of preeclampsia(P<0.05).There was no statistical significance between the two groups in the first onset week,the termination week,systolic blood pressure,diastolic blood pressure,and the history of diabetes(P>0.05).The results of multiple logistic regression analysis showed that the age of initial onset≥35 years,the interval between pregnancies≥5 years,pre-pregnancy obesity or overweight,and a family history of preeclampsia were related factors for the recurrence of preeclampsia in secondary pregnancy(OR>1,P<0.05).Conclusion The age of initial onset≥35 years,the interval between pregnancies≥5 years,pre-pregnancy obesity or overweight,and a family history of preeclampsia are related factors for the recurrence of preeclampsia in secondary pregnant women.
