糖化白蛋白在2型糖尿病诊断中的临床实用性

The clinical practicability of glycated albumin in the diagnosis of diabetes

目的探讨糖化白蛋白(GA)在2型糖尿病(T2DM)的临床诊断中的价值和实用性。方法选取2020年1月至2021年1月于本院内分泌科就诊T2DM患者300例作为研究对象,检测所有患者GA,均行空腹血糖(FBG)和糖耐量试验,记录阳性研究对象人数。以口服糖耐量试验(OGTT)为T2DM诊断金标准,分析GA对T2DM诊断的灵敏度、特异度、符合率、阳性预测值、阴性预测值、约等指数。结果GA检测阳性256例,占比85.33%;FBG阳性278例,占比92.67%;OGTT结果显示,2 h血糖阳性281例,占比93.67%,3种检测方法结果比较差异无统计学意义。GA作为诊断T2DM的指标其灵敏度为90.91%,特异度为76.00%,约登指数为66.91%,符合率为89.67%,阳性预测值为97.67%,阴性预测值为43.18%。结论GA作为诊断T2DM的指标具有灵敏度、特异度高,可靠性好,可作为筛检诊断T2DM的指标,临床价值较高。

Objective To explore the value and practicability of glycated albumin(GA)in the clinical diagnosis of type 2 diabetes mellitus(T2DM).Methods A total of 300 patients with T2DM who were treated in the department of endocrinology our hospital from January 2020 to January 2021 were selected as the research subjects,Glycated albumin was detected in all patients,fasting blood glucose(FBG)and glucose tolerance test were performed,and the number of positive subjects was recorded.Using oral glucose tolerance test(OGTT)as the gold standard for T2DM diagnosis,the sensitivity,specificity,coincidence rate,positive predictive value,negative predictive value and approximate index of GA for T2DM diagnosis were analyzed.Results Among 300 patients,256 cases were positive in GA test,accounting for 85.33%;FBG was positive in 278 cases,accounting for 92.67%;oral glucose tolerance test(OGTT)2 h blood glucose was positive in 281 cases,accounting for 93.67%.There was no significant difference in the results of the three detection methods.The sensitivity,specificity,correct index,coincidence rate,positive predictive value and negative predictive value of GA in the diagnosis of T2DM were 90.91%,76.00%,66.91%,89.67%,97.67%and 43.18%,respectively.Conclusion GA as an indicator for the diagnosis of T2DM has high sensitivity,high specificity and good reliability.As an indicator for screening and diagnosis of T2DM,GA has higher clinical value.
