补体C1q肿瘤坏死因子相关蛋白1与老年原发性高血压患者微量白蛋白尿的相关性研究

Correlation Between Complement C1q Tumor Necrosis Factor Related Protein-1 and Microalbuminuria in Elderly Patients with Essential Hypertension

目的探讨补体C1q肿瘤坏死因子相关蛋白1(CTRP1)与老年原发性高血压患者微量白蛋白尿(MAU)的关系。方法选取2020年7月至2022年7月本院收治的80例老年原发性高血压患者,入院后检测所有患者24 h尿液,根据尿白蛋白与肌酐比值(UACR),将患者分为两组,合并MAU(UACR 30~300 mg/g)为观察组(44例)、正常MAU(UACR<30 mg/g)为对照组(36例)。测定两组CTRP1、肿瘤坏死因子-ɑ(TNF-α)、脂联素、尿白蛋白肌酐比(UACR)、反应蛋白(CRP)及血脂代谢等水平,并分析CTRP1与老年原发性高血压患者MAU的关系。结果观察组CTRP1、TNF-α、UACR水平比对照组高,脂联素水平比对照组低,差异有统计学意义(P<0.05);经Pearson相关性分析显示,CTRP1与TNF-α、UACR水平呈正相关,与脂联素呈负相关,差异有统计学意义(P<0.05)。多元线性回归分析结果显示,CTRP1、TNF-α及脂联素是老年性高血压患者UACR的影响因素,差异有统计学意义(P<0.05)。结论CTRP1、TNF-α及脂联素在老年原发性高血压患者MAU发生与发展中具有重要作用。

Objective To investigate the relationship between complement C1q tumor necrosis factor associated protein 1(CTRP1)and microalbuminuria(MAU)in elderly patients with essential hypertension.Methods From July 2020 to July 2022,80 elderly patients with essential hypertension admitted to our hospital were selected.After admission,the urine of all patients was measured for 24 hours.According to the ratio of urinary albumin to creatinine(UACR),the patients were divided into the observation group(44 cases)with combined MAU(UACR 30 to 300 mg/g)and the control group(36 cases)with normal MAU(UACR<30 mg/g).Collect general data of all patients and measure CTRP1 and tumor necrosis factor in both groups-ɑ(TNF-α)、Adiponectin,urinary albumin creatinine ratio(UACR),reactive protein(CRP),and lipid metabolism.The relationship between CTRP1 and MAU in elderly patients with essential hypertension was analyzed.Results CTRP1,TNF in the observation group-α、The level of UACR was higher than that of the control group,while the level of adiponectin was lower than that of the control group,with significant differences(P<0.05);Pearson correlation analysis showed that CTRP1 and TNF-α、The level of UACR was positively correlated(P<0.05)and negatively correlated with adiponectin(P<0.05).Multiple linear regression analysis showed that CTRP1,TNF-αAnd adiponectin were the influencing factors of UACR in elderly hypertensive patients(P<0.05).Conclusion CTRP1,TNF-αand adiponectin play an important role in the occurrence and development of MAU in elderly patients with essential hypertension.
