中药自拟方加减治疗脑血管性头痛的临床效果观察

Clinical observation on the effect of modified self-made herbal medicine on cerebrovascular headache

目的观察中药自拟方加减治疗脑血管性头痛的临床效果。方法256例脑血管性头痛患者,依据治疗方案不同分为对照组(120例)和治疗组(136例)。对照组以西医扩张血管、改善循环治疗,治疗组在对照组治疗基础上采用中药自拟血府逐瘀汤加减治疗。比较两组患者治疗前后头痛发作疼痛评分、头痛发作次数、头痛持续时间、睡眠障碍评分以及临床疗效。结果治疗后,两组患者头痛发作疼痛评分与睡眠障碍评分降低,头痛发作次数减少,头痛持续时间缩短,且治疗组患者头痛发作疼痛评分(0.81±0.22)分、睡眠障碍评分(3.87±0.55)分低于对照组的(1.22±0.45)、(6.80±1.10)分,头痛发作次数(0.24±0.06)次/个月少于对照组的(0.87±0.21)次/个月、头痛持续时间(3.27±1.36)min/次短于对照组的(5.49±2.58)min/次,差异均具有统计学意义(P<0.05)。治疗组患者治疗总有效率98.53%高于对照组的89.17%,差异具有统计学意义(P<0.05)。结论在脑血管性头痛患者中以中药自拟方加减治疗,可显著改善患者头痛发作时的疼痛程度,减少发作次数,缩短疼痛持续时间,并缓解睡眠障碍,提升临床治疗效果。

Objective To observe the clinical effect of modified self-made herbal medicine on cerebrovascular headache.Methods A total of 256 patients with cerebrovascular headache were divided into control group(120 cases)and treatment group(136 cases)according to different treatment regiments.The control group was treated with Western medicine to dilate blood vessels and improve circulation,and the treatment group was treated with modified Xuefu Zhuyu Decoction of traditional Chinese medicine based on the control group.The score of headache attack pain,the number of headache attack,the duration of headache,the score of sleep disorder before and after treatment,and the clinical efficacy of the two groups were compared.Results After treatment,the score of headache attack pain and sleep disorder and the number of headache attacks in both groups were reduced,and the duration of headache was shortened;the score of headache attack pain of(0.81±0.22)points and score of sleep disorder of(3.87±0.55)points in the treatment group were lower than those of(1.22±0.45)and(6.80±1.10)points in the control group;the number of headache attacks of(0.24±0.06)attacks/month in the treatment group was less than that of(0.87±0.21)attacks/month in the control group;the duration of headache of(3.27±1.36)min/time in the treatment group was shorter than that of(5.49±2.58)min/time in the control group;the differences were statistically significant(P<0.05).The total effective rate of the treatment group was 98.53%,which was higher than that of 89.17%of the control group,and the difference was statistically significant(P<0.05).Conclusion In patients with cerebrovascular headache,the modified self-made Chinese herbal medicine can significantly improve the pain level in patients with headache attacks,reduce the number of attacks,shorten the duration of pain,relieve sleep disorders,and improve the clinical treatment effect.
