温针灸联合穴位贴敷治疗寒湿痹阻型膝关节骨性关节炎的效果探究

Study on the effect of warm acupuncture combined with acupoint application in the treatment of knee osteoarthritis with cold-dampness obstruction syndrome

目的观察温针灸联合穴位贴敷治疗寒湿痹阻型膝关节骨性关节炎(KOA)的效果。方法70例寒湿痹阻型膝关节骨性关节炎患者,采用随机数字表法分为观察组和对照组,每组35例。观察组采用温针灸联合穴位贴敷治疗,对照组给予布洛芬缓释胶囊治疗。比较两组患者视觉模拟评分法(VAS)评分、骨关节炎指数(WOMAC)评分、膝关节疼痛消失时间及治疗效果。结果治疗前,两组患者VAS评分比较,差异无统计学意义(P>0.05);治疗后7、14 d,观察组患者VAS评分分别为(3.56±0.27)、(1.34±0.20)分,均明显低于对照组的(5.34±0.33)、(2.65±0.24)分,差异具有统计学意义(P<0.05)。治疗前,两组患者WOMAC评分比较,差异无统计学意义(P>0.05);治疗后14 d,观察组患者WOMAC评分(23.44±2.40)分明显低于对照组的(35.01±2.15)分,差异具有统计学意义(P<0.05)。观察组患者膝关节疼痛消失时间(10.87±1.42)d短于对照组的(13.16±1.50)d,差异具有统计学意义(P<0.05)。观察组患者治疗总有效率91.4%高于对照组的71.4%,差异具有统计学意义(P<0.05)。结论温针灸联合穴位贴敷能明显改善寒湿痹阻型膝关节骨性关节炎患者的临床症状及关节功能,缓解疼痛,值得推广。

Objective To observe the effect of warm acupuncture combined with acupoint application in the treatment of knee osteoarthritis(KOA)with cold-dampness obstruction syndrome.Methods A total of 70 cases of knee osteoarthritis with cold-dampness obstruction syndrome were divided into observation group and control group according to the random numerical table,with 35 cases in each group.The observation group was treated with warm acupuncture and acupoint application,and the control group was treated with ibuprofen sustained release capsule.The visual analogue scale(VAS)score,Western Ontario and McMaster Universities Arthritis Index(WOMAC)score,disappearance time of knee pain,and the therapeutic effect were compared between the two groups.Results Before treatment,there was no statistically significant difference in VAS scores between the two groups(P>0.05).After 7 and 14 d of treatment,VAS scores of the observation group were(3.56±0.27)and(1.34±0.20)points,which were significantly lower than those of(5.34±0.33)and(2.65±0.24)points of the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in WOMAC score between the two groups(P>0.05).After 14 d of treatment,WOMAC score of(23.44±2.40)points in the observation group was significantly lower than that of(35.01±2.15)points in the control group,and the difference was statistically significant(P<0.05).The disappearance time of knee pain of(10.87±1.42)d in the observation group was shorter than that of(13.16±1.50)d in the control group,and the difference was statistically significant(P<0.05).The total effective rate of 91.4%in the observation group was higher than that of 71.4%in the control group,and the difference was statistically significant(P<0.05).Conclusion Warm acupuncture combined with acupoint application can obviously improve the clinical symptoms and joint function of KOA with cold-dampness obstruction syndrome,alleviate the pain of patients,which is worthy of promotion.
