血清ESM-1、P-selectin、Galectin-3与急性心肌梗死病情关系及预测预后价值

Association of serum ESM-1,P-selectin and Galectin-3 with AMI and predictive prognostic value

摘要 目的 探讨血清内皮细胞特异性分子1(ESM-1)、P选择素(P-selectin)、半乳糖凝集素-3(Galectin-3)与急性心肌梗死(AMI)病情程度关系及对预后不良的预测价值。方法 选取成都市第八人民医院2019年1月—2021年6月AMI患者119例作为研究对象,随访3个月统计预后,分为预后良好组(81例)与预后不良组(38例)。比较两组临床资料以及血清ESM-1、P-selectin、Galectin-3水平,分析上述指标与病情程度的关系,并评价其预测预后价值。结果 预后不良组血清ESM-1、P-selectin、Galectin-3水平高于预后良好组(P<0.05);血清ESM-1、P-selectin、Galectin-3水平随病情程度加重呈逐渐升高趋势,与病情程度呈正相关(P<0.05);血清ESM-1、P-selectin、Galectin-3与预后不良风险显著相关(P<0.05);血清ESM-1、P-selectin、Galectin-3联合预测预后不良的AUC为0.889(95%CI:0.818~0.939),敏感度为86.84%,特异度为83.95%,较各指标单独诊断价值提高。结论 血清ESM-1、P-selectin、Galectin-3与AMI病情程度呈正相关,联合预测预后不良价值较为可靠,可作为临床评估病情、预测预后的潜在指标。 Objective To investigate the relationship between serum endothelial cell specific molecule 1(ESM-1),Pselectin(P-selectin),galectin-3(Galectin-3)and the severity of acute myocardial infarction(AMI)and the prediction of poor prognosis value.Methods 119 patients with AMI from January 2019 to June 2021 in the Eighth People's Hospital of Chengdu were selected as the study subjects,and the prognosis was counted during the 3-month follow-up period and divided into the good prognosis group(81 cases)and the poor prognosis group(38 cases).The clinical data,serum ESM-1,P-selectin and Galectin-3 levels were compared between the two groups,and the relationship between the above indicators and the degree of disease was analysed and their predictive prognostic value was evaluated.Results Serum ESM-1,P-selectin and Galectin-3 levels were higher in the poor prognosis group than in the good prognosis group(P<0.05);serum ESM-1,P-selectin and Galectin-3 levels tended to increase gradually with increasing severity of the disease and were positively correlated with the severity of the disease(P<0.05).Serum ESM-1,P-selectin and Galectin-3 were significantly as sociated with the risk of poor prognosis(P<0.05).The AUC ofserum ESM-1,P-selectin,and Galectin-3 combined to predict poor prognosis was 0.889(95%CI:0.818-0.939),with a sensitivity of 86.84%and a specificity of 83.95%,which was higher than the diagnostic value of each indicator alone.Conclusion Serum ESM-1,P-selectin and Galectin-3 are positively correlated with the severity of AMI,and the combined value of predicting poor prognosis is reliable,and can be used as potentia lindicators for clinical assessment of disease and prognosis.
