三维经食管超声心动图评估非瓣膜性心房颤动患者左心耳封堵术后二尖瓣装置形态

Evaluation of mitral valve device morphology by three-dimensional transesophageal echocardiography in patients with nonvalvular atrial fibrillation after left atrial appendage occlusion

摘要 目的 应用三维经食管超声心动图(3D-TEE)评估非瓣膜性心房颤动(以下简称房颤)患者左心耳封堵术后二尖瓣装置形态,探讨其临床应用价值。方法 选取于我院行左心耳封堵术治疗的非瓣膜性房颤患者35例,分别于手术前及手术后8周行3D-TEE检查,比较手术前后二尖瓣装置形态参数即瓣环前后径、瓣环前外侧至后内侧径、瓣环前后径与前外侧至后内侧径比值、瓣环非平面角度、三维瓣环周长、二维瓣环面积、三维瓣环面积、瓣环高度、瓣叶穹隆容积、瓣叶穹隆下投影面积、瓣叶穹隆高度、瓣叶联合处直径、前叶面积、后叶面积、前叶长度、后叶长度、后叶角度、远端前叶角度变化情况;分析二尖瓣装置形态参数与术后封堵器压缩比的相关性。结果 与术前比较,术后瓣环前外侧至后内侧径、三维瓣环周长、二维瓣环面积、三维瓣环面积、瓣叶穹隆容积、瓣叶穹隆下投影面积及瓣叶联合处直径均减少,瓣环前后径与前外侧至后内侧径比值增大,差异均有统计学意义(均P<0.05);其余参数术前与术后比较差异均无统计学意义。术后封堵器压缩比与瓣叶穹隆容积呈正相关(r=0.44,P<0.05),与二尖瓣装置其他形态参数均无相关性。结论 3D-TEE可准确评估左心耳封堵术后二尖瓣装置形态改变,可为术前封堵器选择提供更多参考。 Objective To evaluate the mitral valve device morphology by three-dimensional transesophageal echocardiography(3D-TEE)in patients with nonvalvular atrial fibrillation after left atrial appendage occlusion,and to explore its clinical application value.Methods Thirty-five patients with nonvalvular atrial fibrillation who underwent left atrial appendage occlusion in our hospital were examined by 3D-TEE before and after 8 weeks of surgery.The parameters of mitral valve devices morphology before and after surgery were compared,including anterior-anterolateral to posteriormedial diameter,anterolateral to posteriormedial diameter,the ratio of anterior-anterolateral to posteriormedial diameter and anterolateral to posteriormedial diameter,nonplane angle,three-dimensional annulus circumference,two-dimensional annulus area,three-dimensional annulus area,annulus height,dome volume,subdome area,dome height,diameter at the joint of lobes,anterior lobe area,posterior lobe area,anterior lobe length,posterior lobe length,posterior lobe angle and distal anterior lobe angle.The correlation between compression ratio of occluder and quantitative parameters of mitral valve device was analyzed.Results Compared with the preoperative results,the anterolateral to posterolateral diameter,three-dimensional annulus circumference,two-dimensional annulus area,three-dimensional annulus area,dome volume,subdome area,diameter at the joint of lobes decreased,and the ratio of anterior-anterolateral to posteriormedial diameter and anterolateral to posterolateral diameter increased after operation,with statistical significances(all P<0.05).There were no significant differences in other parameters between preoperative and postoperative results.The compression ratio of the occluder was positively correlated with dome volume(r=0.44,P<0.05),and not correlated with other parameters of the mitral valve device.Conclusion 3D-TEE can accurately evaluate the morphological changes of mitral valve device after left atrial appendage closure,and provide more references for the selection of preoperative occluders.
