阶段性体位干预在口腔癌术后患者中的应用效果

Application of staged posture nursing in patients after oral cancer surgery

目的探讨阶段性体位干预在口腔癌术后患者中的应用效果。方法选取2021年6月-2022年12月笔者所在医院收治的200例口腔癌行游离组织瓣移植修复重建术患者为研究对象,采用随机数字表法将其分为对照组和观察组,各100例。对照组实施口腔癌术后传统体位护理,观察组实施阶段性体位干预。比较2组术后并发症、舒适度及术后康复进程。结果干预后,观察组压力性损伤发生率低于对照组(χ^(2)=4.189,P=0.041);术后首次下床时间及术后住院时间均短于对照组(t=9.755,P<0.001;t=2.642,P=0.003)。2组舒适度得分在时间效应、组间效应及交互效应方面比较,差异均有统计学意义(F时间=1539.670,P<0.001;F组间=124.850,P<0.001;F交互=25.197,P<0.001);2组血管危象发生例数比较,差异无统计学意义(χ^(2)=0.096,P=0.756)。结论阶段性体位干预能有效降低口腔癌术后患者压力性损伤发生率,提高患者舒适度,促进患者术后康复。

Objective To explore the effect of phased posture intervention on postoperative patients with oral cancer.Methods Totally 200 oral cancer patients who underwent free tissue flap transplantation and reconstruction between June 2021 and December 2022 were selected,and divided into an observation group and a control group,each of 100,according to a random number table.The control group received routine posture nursing after oral cancer surgeries,while the observation group underwent staged posture intervention.The postoperative complications,comfort and recovery were compared between the two groups.Results After the intervention,the incidence of pressure injury in the observation group was significantly lower than that of the control group(χ^(2)=4.189,P=0.041).The comfart scores of the two groups were compared in the aspects of time effect,intergroup effect and interaction effect,the differences were statistically significant(F time=1539.670,P<0.001;F intergroup=124.850,P<0.001;F interaction=25.197,P<0.001).Moreover,the time of getting out of bed in the former group was significantly earlier than the latter,while the postoperative hospital stay was significantly shorter than the latter(t=9.755,P<0.001;t=2.642,P=0.003).There was no signifieant difference in the numbers of cases of vascular erisis between the two groups(χ^(2)=0.096,P=0.756).Conclusion Phased posture intervention can effectively lower the incidence of stress injury of patients with oral cancer after surgeries,as well as improve their comfort and rehabilitation.
