血府逐瘀汤加减结合理筋手法对乳腺癌术后上肢水肿及活动障碍患者的影响

Therapeutic Effect of Xue Fu Zhu Yu Tang Decoction Combined with Tendon-Massage on Patients with Upper Extremity Edema and Mobility disorders after Breast Cancer Surgery

目的:观察血府逐瘀汤加减结合理筋手法对乳腺癌术后上肢水肿及活动障碍的影响。方法:选取2021年2月—2023年2月中山大学孙逸仙纪念医院中医科收治的乳腺癌术后出现上肢水肿及活动障碍女性患者120例为研究对象,按随机分组法分为两组,各60例。两组患者均进行功能锻炼,对照组行理筋手法,治疗组在对照组基础上给予血府逐瘀汤加减治疗。比较两组上肢水肿程度及上肢功能、生活质量、临床疗效。结果:治疗后,两组患者上肢周径差值、上肢功能评定表评分均下降,Fugl-Meyer运动功能评定量表上肢评分和整体生活质量评分均上升,且治疗组优于对照组,差异有统计学意义(P<0.05);治疗后,两组患者整体生活质量评分均升高,疼痛与睡眠障碍评分均下降,且治疗组各项评分均优于对照组,差异有统计学意义(P<0.001);治疗组总有效率高于对照组,差异有统计学意义(P=0.028)。结论:血府逐瘀汤加减结合理筋手法可有效改善乳腺癌术后患者上肢水肿和活动障碍,提高其生活质量,临床疗效较好。

Objective:To observe the therapeutic effects of Xue Fu Zhu Yu Tang Decoction combined with tendon-Massage on the edema of the upper limbs and reduced mobility after breast cancer surgery.Methods:120 female patients with upper limb edema and reduced mobility after breast cancer surgery admitted to the Department of Traditional Chinese Medicine of Sun Yat-sen Memorial Hospital of Sun Yat-sen University from February 2021 to February 2023 were selected as the study subjects,and were divided into two groups of 60 patients each according to the randomized grouping method.Physical exercises were carried out in both groups.The control group was treated also with Tendon-massage,while the treatment group was treated with Xue Fu Zhu Yu Tang Decoction on the basis of the control group.The two groups were compared in terms of the degree of upper limb edema,upper limb function,quality of life and clinical efficacy.Results:after treatment the upper extremity circumference difference and upper extremity functional score of both groups decreased,and the upper extremity score of Fugl-Meyer Assessment and the overall quality of life score of the two groups increased,and the treatment group was better than the control group,and the difference was statistically significant(P<0.05);after treatment,the overall quality of life score of both groups increased,and the pain and sleep disorder score decreased,and the treatment group was better than the control group in all scores.After treatment,the overall quality of life scores of both groups increased,and the pain and sleep disorder scores decreased,and the treatment group was better than the control group,with a statistically significant difference(P<0.001);the total effective rate of the treatment group was higher than that of the control group,with a statistically significant difference(P=0.028).Conclusion:The combination of Xue Fu Zhu Yu Tang Decoction and Tendon-Massage can effectively improve the upper limb edema and reduced mobility of postoperative breast cancer patients,improve their quality of life,and have better clinical efficacy.
