脑电双频指数监测下使用丙泊酚、盐酸戊乙奎醚联合右美托咪定在小儿尿动力学检查中的应用价值

摘要 目的:分析脑电双频指数(BIS)监测下使用丙泊酚、盐酸戊乙奎醚联合右美托咪定在小儿尿动力学检查中的效果。方法:选取2020年12月—2023年1月东莞市第八人民医院收治的行小儿尿动力学检查的患儿60例为观察对象,根据随机数字表法分为对照组和观察组,各30例。对照组给予传统口服水合氯醛镇静,观察组给予BIS监测下静脉使用丙泊酚、盐酸戊乙奎醚联合右美托咪定镇静。比较两组心率(HR)、平均动脉压(MAP)、血氧饱和度(SPO2)值,不良反应发生情况及术者满意度。结果:检查操作开始时、膀胱充盈期及排尿期,观察组HR、MAP值低于对照组,SPO2值高于对照组,差异有统计学意义(P<0.05);观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.044);观察组术者总满意度高于对照组,差异有统计学意义(P=0.037)。结论:BIS监测下静脉使用丙泊酚、盐酸戊乙奎醚、右美托咪定镇静可以提高术者满意度,降低不良反应发生率,使患儿血流动力学更稳定。 Objective:To analyze the effect of intravenous use of propofol and penehyclidine hydrochloride combined with dexmedetomidine in pediatric urodynamics under electroencephalographic dual-frequency index(BIS)monitoring.Methods:Sixty cases of children admitted to Dongguan Eighth People's Hospital for pediatric urodynamic examination from December 2020 to January 2023 were selected as observation subjects,and were divided into a control group and an observation group of thirty cases each according to the method of randomized numerical table.The control group was given traditional oral chloral hydrate sedation,and the observation group was given intravenous propofol,penehyclidine hydrochloride combined with dexmedetomidine sedation under BIS monitoring.Heart rate(HR),mean arterial pressure(MAP),blood oxygen saturation(SPO2)values,occurrence of adverse reactions and operator satisfaction were compared between the two groups.Results:At the beginning of the examination operation,bladder filling period and voiding period,HR and MAP values of the observation group were lower than those of the control group,and SPO2 values were higher than those of the control group,and the difference was statistically significant(P<0.05);The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.044);The total satisfaction of operators in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.037).Conclusion:Intravenous sedation with propofol,penehyclidine hydrochloride,and dexmedetomidine under BIS monitoring can improve operator satisfaction,reduce the incidence of adverse reactions,and make children's hemodynamics more stable.
