中药内服外敷联合胫骨横向骨搬运术治疗糖尿病足临床效果

Clinical effects of diabetic foot treated by internal and external applications of traditional Chinese medicine combined with tibial transverse transport

目的探讨中药内服外敷联合胫骨横向骨搬运术治疗糖尿病足的临床效果。方法采用回顾性分析,观察遂宁市中医院骨一科2019年10月至2021年10月共17例采用中药内服外敷联合胫骨横向骨搬运术治疗的糖尿病足患者的疼痛视觉模拟评分法(VAS)、患肢足部皮温、超敏C反应蛋白(hs-CRP)、手术前后患肢的动脉血管造影、糖尿病足的痊愈率及患肢的截肢情况等指标,分析其临床疗效。结果术后第1、2、4周的VAS评分、患肢足部皮温、hs-CRP水平均较术前明显好转,差异有统计学意义(P<0.05)。术后患肢动脉血管造影均提示较术前明显改善,患肢痊愈率、保肢率较高。结论糖尿病足患者采用中药内服外敷联合胫骨横向骨搬运术能够提高保肢率。

Objective To explore the clinical effects of internal and external applications of traditional Chinese medicine combined with tibial transverse transport in the treatment of diabetic foot.Methods Retrospective analysis was used to observe the pain score of the visual analogue scale(VAS),skin temperature of affected limbs and feet,hypersensitive C-reactive protein(hs-CRP),arteriography of affected limbs before and after the surgery,recovery rate of diabetic foot,amputation of affected limbs and other indicators of 17 patients with diabetic foot who were treated by internal and external applications of traditional Chinese medicine combined with tibial transverse transport from October 2019 to October 2021 in the DepartmentⅠof Orthopaedics of Suining Municipal Hospital of Traditional Chinese Medicine,and analyze its clinical efficacy.Results After treatment,the patient’s VAS score,affected limb foot skin temperature,and the level of hs-CRP were significantly improved in the first week,second week,and fourth week after surgery compared to before surgery,with statistically significant differences(P<0.05).Postoperative arterial angiography of the affected limb showed significant improvement compared to before surgery,with a higher recovery rate and limb salvage rate.Conclusion The combination of internal and external applications of traditional Chinese medicine and tibial transverse transport can improve limb salvage rate in patients with diabetic foot.
