多维度中西医结合预康复对重度-极重度COPD患者运动耐力、生活质量改善影响

The impact of multi-dimensional integrated Chinese and Western medicine pre-rehabilitation on the improvement of exercise endurance and quality of life in patients with severe to very severe COPD

目的探究多维度中西医结合预康复对重度-极重度慢性阻塞性肺疾病(COPD)急性加重期后患者运动耐力生活质量及肺功能的影响。方法选取2019年1月至2020年12月广东省中西医结合医院呼吸内科收治的60例重度-极重度COPD急性加重期患者,根据随机数表法分为预康复组和对照组,每组各30例。对照组采用常规的急性加重期西医治疗,预康复组自住院入组日起采用中西医结合预康复策略。比较两组患者在治疗前后的运动耐力、生活质量以及肺功能的影响。结果预康复组的运动耐力评估、肺功能、生活质量评估及气促症状评估均较对照组提升显著,差异有统计学意义(P<0.05)。结论多维度中西医结合预康复对重度-极重度COPD患者临床运用安全有效,尤其对于急性加重期患者可以显著提高急性加重期COPD患者运动耐受力,提升生活质量,可极大促进患者早期肺功能康复及社会功能恢复。

Objective To explore the effects of multi-dimensional integrated Chinese and Western medicine pre-rehabilitation on exercise endurance,quality of life,and pulmonary function in patients with severe to very severe chronic obstructive pulmonary disease(COPD)after acute exacerbation.Methods A total of 60 patients with acute exacerbation of severe to very severe COPD admitted to the Department of Respiratory Medicine of Guangdong Hospital of Integrated Chinese and Western Medicine from January 2019 to December 2020 were selected and divided into a pre-rehabilitation group and a control group using a random number table method,with 30 patients in each group.The control group was treated with conventional acute exacerbation Western medicine,while the pre-rehabilitation group was treated with a pre-rehabilitation strategy of integrated Chinese and Western medicine from the date of admission.The effects of exercise endurance,quality of life,and pulmonary function between the two groups of patients before and after treatment were compared.Results The exercise endurance assessment,pulmonary function,quality of life assessment,and shortness of breath symptom assessment in the pre-rehabilitation group were significantly improved compared to the control group,with statistically significant differences(P<0.05).Conclusion Multi-dimensional integrated Chinese and Western medicine pre-rehabilitation is safe and effective for severe to very severe COPD patients in clinical practice,especially for patients with acute exacerbation of COPD,which can significantly improve exercise endurance and quality of life of COPD patients in acute exacerbation,and greatly promote early pulmonary function recovery and social function recovery of patients.
