某院2019-2021年门诊药房抗高血压药品使用情况分析

Analysis on the use of antihypertensive drugs in the outpatient pharmacy of the hospital from 2019 to 2021

目的调查分析复旦大学附属中山医院厦门医院(本院)门诊药房的抗高血压药的处方,为医生在临床上合理用药提供参考,使药师能够规范合理、安全有效地指导患者使用药物。方法利用药房HIS系统提取2019年1月至2021年12月门诊高血压患者使用抗高血压药品数据,通过使用世界卫生组织推荐的限定日剂量(DDD),由此计算用药频度(DDDs)、药物的日均费用(DDC)及排序比(B/A)等指标。结果2019年1月至2021年12月本院门诊处方中使用的各类抗高血压药物的治疗方案基本合理,主要使用的药物种类有钙通道拮抗剂(CCB)、血管紧张素Ⅰ转化酶抑制剂(ACEⅠ)、β-受体拮抗剂、血管紧张素Ⅱ受体拮抗剂(ARB)、利尿药、复方制剂等,使用频率最高的药物是CCB、ARB和复方制剂。结论本院各类抗高血压药应用的研究内容基本符合2018年修订版《中国高血压防治指南》,联合用药情况基本符合标准。

Objective To investigate and analyze the prescriptions of antihypertensive drugs in the outpatient pharmacy of Xiamen Branch(our hospital),Zhongshan Hospital,Fudan University so as to provide references for doctors to use drugs rationally in clinical practice,and enable pharmacists to guide patients to use drugs in a standardized,reasonable,safe and effective manner.Methods The pharmacy HIS system was used to extract the data on antihypertensive drugs used by outpatients with hypertension from January 2019 to December 2021.By using the defined daily dose(DDD)recommended by the World Health Organization,the defined daily doses(DDDs),average defined daily cost(DDC)and ranking ratio(B/A)of drugs were calculated.Results From January 2019 to December 2021,the treatment scheme of various antihypertensive drugs used in our outpatient prescriptions is basically reasonable,and the main drugs used are calcium channel blockers(CCB),angiotensinⅠconverting enzyme inhibitors(ACEⅠ),β-receptor antagonists,angiotensinⅡreceptor antagonists(ARB),diuretics,polypill,etc.The drugs with the highest frequency of use are CCB,ARB,and polypill.Conclusion The research content of the application of various antihypertensive drugs in our hospital is basically in line with the 2018 Chinese Guidelines for Prevention and Treatment of Hypertension(Revision),and the situation of combined drug use is basically in line with the standards.
