血清闭锁蛋白和闭锁小带蛋白1表达与高血压脑出血后认知功能障碍的相关性分析

摘要 目的探究血清闭锁蛋白和闭锁小带蛋白1表达与高血压脑出血后认知功能障碍的相关性分析。方法选取2019年1月至2022年12月江苏省扬州市第二人民医院神经外科纳入的60例高血压脑出血患者,采用蒙特利尔认知评估量表(MoCA)评估认知功能,然后根据MoCA评分结果分为认知功能障碍组(<26分,n=28)和认知功能正常组(≥26分,n=32)。采用Pearson法对血清闭锁蛋白和闭锁小带蛋白1水平与高血压脑出血后认知功能障碍的相关性进行分析,并采用多因素logistic回归模型对影响高血压脑出血后认知功能障碍的危险因素进行分析。结果认知功能障碍组血清闭锁蛋白和闭锁小带蛋白1水平以及MoCA评分均低于认知功能正常组(P<0.05);认知功能障碍组糖尿病史、高血压史、饮酒史占比高于认知功能正常组(P<0.05);认知功能障碍组性别、年龄、低密度脂蛋白、三酰甘油等其他临床资料与认知功能正常组比较,差异无统计学意义(P>0.05)。高血压脑出血患者血清闭锁蛋白、闭锁小带蛋白1水平与MoCA评分均呈正相关(r=0.611,P=0.001;r=0.602,P=0.001)。进行多因素logistic回归模型分析,低水平血清闭锁蛋白和闭锁小带蛋白1是导致高血压脑出血患者认知功能障碍的主要独立危险因素(P<0.05)。结论高血压脑出血后认知功能障碍患者血清闭锁蛋白和闭锁小带蛋白1水平均偏低,且血清闭锁蛋白和闭锁小带蛋白1水平与高血压脑出血后认知功能障碍呈现一定的相关性,是影响高血压脑出血后认知功能障碍的独立危险因素,可作为高血压脑出血临床辅助诊断指标。 Objective To explore the correlation between the expression of serum occludin and zonula occludens-1 and cognitive dysfunction after hypertensive cerebral hemorrhage.Methods A total of 60 patients with hypertensive cerebral hemorrhage admitted to the Department of Neurosurgery,the Second People’s Hospital of Yangzhou City,from January 2019 to December 2022 were selected.Their cognitive function was evaluated by Montreal cognitive assessment(MoCA),and then they were divided into the cognitive dysfunction group(score<26,n=28)and the normal cognitive function group(score≥26,n=32)according to the MoCA score.Pearson method was adopted to analyze the correlation between the levels of serum occludin and zonula occludens-1 and cognitive dysfunction after hypertensive cerebral hemorrhage,and multivariate logistic regression model was adopted to analyze the risk factors of cognitive dysfunction after hypertensive cerebral hemorrhage.Results The serum levels of occluding and zonula occludens-1 and MoCA score in the cognitive dysfunction group were lower than those in normal cognitive function group(P<0.05).The history of diabetes,hypertension and drinking in the cognitive impairment group was significantly different from that in the normal cognitive function group(P<0.05).There was no difference in gender,age,low-density lipoprotein cholesterol,triacylglycerol,and other clinical data between the cognitive dysfunction group and the normal cognitive function group(P>0.05).There was significantly positive correlation between the levels of serum occludin and zonula occludens-1 and the MoCA score in patients with hypertensive cerebral hemorrhage(r=0.611,P=0.001.r=0.602,P=0.001).Through multivariate logistic regression model analysis,it was found that low levels of serum occludin and zonula occludens-1 were the main independent risk factors for cognitive dysfunction in patients with hypertensive cerebral hemorrhage(P<0.05).Conclusion The levels of serum occludin and zonula occludens-1 in patients with cognitive dysfunction after hypertensive cerebral hemorrhage are low,and the levels of serum occludin and zonula occludens-1 are correlated with cognitive dysfunction after hypertensive cerebral hemorrhage,which are independent risk factors for cognitive dysfunction after hypertensive cerebral hemorrhage.Therefore,the levels of serum occludin and zonula occludens-1 can be used as clinical auxiliary diagnostic indicators for hypertensive cerebral hemorrhage.
