分析富马酸卢帕他定治疗慢性自发性荨麻疹的效果及对外周血T淋巴细胞亚群、睡眠质量的影响

To Analyze the Effect of Lupatadine Fumarate on Chronic Spontaneous Urticaria and Its Effect on Peripheral Blood T Lymphocyte Subsets and Sleep Quality

目的:分析富马酸卢帕他定治疗慢性自发性荨麻疹(CSU)的效果及对外周血T淋巴细胞亚群、睡眠质量的影响。方法:选取2021年9月至2022年12月福建省泉州市皮肤病防治院收治的慢性自发性荨麻疹患者107例作为研究对象,按照随机数字表法分为观察组(n=54)和对照组(n=53)。对照组给予西替利嗪片治疗,观察组给予富马酸卢帕他定治疗。比较2组临床疗效,治疗前后对临床症状进行评分,抽取治疗前及治疗4周后清晨空腹血,测量2组患者免疫功能,采用匹兹堡睡眠质量指数(PSQI)比较2组患者干预前后睡眠质量的变化,并比较2组患者复发及不良反应发生情况。结果:治疗4周后,观察组优良率显著高于对照组,观察组风团发生次数、风团大小、风团持续时间、风团数目、瘙痒程度评分均明显低于对照组,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)明显高于对照组,CD8^(+)明显低于对照组,观察组PSQI评分显著低于对照组,观察组不良反应发生率显著低于对照组,差异均有统计学意义(均P<0.05)。结论:富马酸卢帕他定在CSU治疗中应用较佳,可有效改善患者外周血T淋巴细胞亚群及睡眠质量,降低疾病复发率,提高治疗有效率,可为临床治疗提供科学依据。

Objective:To investigate the effects of lupatadine fumarate on chronic spontaneous urticaria(CSU)and its effects on peripheral blood T lymphocyte subsets and sleep quality.Methods:A total of 107 patients with chronic spontaneous urticaria treated in Quanzhou Hospital of Skin Disease Prevention and Treatment from September 2021 to December 2022 were selected as the study objects,and were divided into observation group(n=54)and control group(n=53)according to random number table method.The control group was treated with cetirizine tablets,and the observation group was treated with lupatadine fumarate.The clinical efficacy of the two groups was compared,clinical symptoms were scored before and after treatment,fasting blood was collected before treatment and 4 weeks after treatment in the morning,and immune function of the two groups was measured.Pittsburgh Sleep Quality Index(PSQI)was used to compare the changes in sleep quality of the two groups before and after intervention,and the recurrence and adverse reactions of the two groups were compared.Results:After 4 weeks of treatment,the excellent and good rate of the observation group was significantly higher than that of the control group.The occurrence times,size,duration,number and pruritus of the observation group were significantly lower than those of the control group.The CD3^(+),CD4^(+),CD4^(+)/CD8^(+)of the observation group were significantly higher than those of the control group,and CD8^(+)was significantly lower than those of the control group.The PSQI score in the observation group was significantly lower than that in the control group,and the incidence of adverse reactions in the observation group was significantly lower than that in the control group,with statistical significance between the two groups(P s<0.05).Conclusion:Lupatadine fumarate is better in the treatment of CSU,which can effectively improve the peripheral blood T lymphocyte subsets and sleep quality of patients,reduce the recurrence rate of disease,improve the treatment efficiency,and provide scientific basis for clinical treatment.
