老年综合评估在2型糖尿病合并睡眠障碍患者治疗中所发挥的应用效果研究

The Application Effect of Geriatric Comprehensive Evaluation in the Treatment of Patients with Type 2 Diabetes Mellitus with Sleep Disorders

目的:分析老年综合评估在2型糖尿病合并睡眠障碍患者治疗中所发挥的应用效果。方法:回顾性分析2022年7月至2022年12月南华医院收治的2型糖尿病合并睡眠障碍患者3751例作为研究对象,根据患者评估管理模式不同将患者划分为对照组(n=1872)与观察组(n=1879)。对照组实施常规评估治疗,观察组实施老年综合评估治疗,比较不同评估模式下组间治疗效果。结果:治疗前,观察组与对照组组间血糖水平、匹兹堡睡眠质量指数(PSQI)及治疗效果比较,差异无统计学意义(P>0.05);治疗3个月后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白低于对照组(均P<0.05);观察组PSQI低于对照组(P<0.05);观察组改良巴氏指数(MBI)、Tinetti量表、简易智力状态检查(MMSE)评分高于对照组,营养风险筛查NRS2002、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于对照组(均P<0.05)。结论:老年综合评估在2型糖尿病合并睡眠障碍患者治疗中综合应用价值更高,能降低患者血糖水平,改善其睡眠质量,在其心理状态、认知功能以及生活能力等维度综合治疗效果更好,建议推广使用。

Objective:To analyze the application effect of geriatric comprehensive evaluation in the treatment of patients with type 2 diabetes and sleep disorders.Methods:The retrospective analysis included 3751 patients from Nanhua hospital with type 2 diabetes and sleep disorders from October 2021 to June 2023.The patients were divided into control group(conventional evaluation treatment,1872 cases)and observation group(geriatric comprehensive evaluation treatment,1879 cases)to compare the treatment effect between groups under different evaluation modes.Results:Before treatment,the differences in blood glucose level,Pittsburgh Sleep Quality Index(PSQI)and treatment effect between the observation group were not significant(P>0.05).After 3 months of treatment,the fasting blood glucose,2-hour postprandial blood glucose,and glycated hemoglobin in the observation group were lower than those in the control group(P s<0.05).PSQI was lower than the control group(P<0.05).Modified Barthel Index(MBI),Tinetti score and Mini-mental State Examination(MMSE)score were higher than the control group,Nutrition Risk Screening(NRS)2002,Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)were lower than the control group(P s<0.05).Conclusion:The geriatric comprehensive evaluation has higher comprehensive application value in the treatment of patients with type 2 diabetes combined with sleep disorders,which can reduce the blood glucose level of patients,improve the sleep quality of patients,and have better comprehensive treatment effect in the dimensions of patients′psychological state,cognitive function and life ability.It is suggested to promote the use.
