基于RBRVS和KPI的超声科绩效模式改革实践

摘要 目的通过绩效改革构建工作量和工作质量考核相结合的绩效薪酬管理模式,充分调动医务人员的工作积极性,鼓励医务人员从事高风险、高难度的技术工作,引导医务人员注重成本控制,提升服务效率,实现科室诊疗和运营水平的全面提升。方法借鉴RBRVS的基本原理,将超声科的操作项目进行量化;然后基于KPI方法,构建质量考核体系;选取某院2021年1月1日-12月31日的诊疗服务数据进行研究。结果超声科的诊疗项目相对劳动价值得到量化,科室的二次分配模式得以丰富,工作效率得到显著提升。服务患者数和服务项目显著提升,门诊检查人次数提高17%,住院检查人次数提高36%;腹部及血管超声提高18%、心脏超声提高11%、阴式超声提高30%、手术超声提高6%、介入超声提高15%;床旁超声服务提高67%。低值耗材使用降低71%。结论RBRVS-KPI的绩效改革模式取得满意效果,改革过程中绩效沟通发挥了重要作用,此种方式的改革具有推广性。 Objectives This study aims to establish a performance-based management model combining workload and work quality assessment through performance reform,so as to fully mobilize the enthusiasm of medical staff,encourage medical staff to engage in high-risk and difficult technical work,guide medical staff to focus on cost control,improve service efficiency,and achieve a comprehensive improvement in the level of departmental diagnosis and operation.Methods The basic principles of RBRVS were used to quantify the operation items of the ultrasound department.Then build a quality assessment system based on the KPI method.The diagnosis and treatment service data of a certain hospital from January 1,2021 to December 31,2021 were selected for research.Results The relative labor value of the diagnosis and treatment items of the ultrasound department was quantified,the secondary distribution mode of the department was enriched,and the work efficiency was significantly improved.The number of patients and service items significantly increased,with a 17%increase in the number of outpatients and 36%of inpatients.Abdominal and vascular ultrasound increased by 18%,cardiac ultrasound increased by 11%,vaginal ultrasound increased by 30%,surgical ultrasound increased by 6%,and interventional ultrasound increased by 15%.Bedside ultrasound services increased by 67%.The use of low-value consumables was reduced by 71%.Conclusions The performance reform model of RBRVS-KPI has achieved satisfactory results,and performance communication has played an important role in the reform process.This reform method has the potential to be promoted.
