护士主导ATSP家庭护理模式在下呼吸道感染患儿中的应用效果分析

Effect analysis of nurse-led ATSP home care model in children with lower respiratory tract infection

目的讨论护士主导的家庭护理模式在儿童下呼吸道感染治疗中的效果。方法选取2020年1月至2020年12月于河北省某儿童医院就诊的下呼吸道感染病例307例,分析门诊和住院治疗患儿的性别、年龄、精神饮食状态、影像学检查、实验室检查等指标。ATSP护理模式包括评估(assess),营养及精神状态;体温管理(temperature management),动态掌握患儿体温情况;高热惊厥(febrile seizure)识别及预防;心理疏导(psychological counseling)等方面,采用独立样本t检验,χ^(2)检验或Fisher精确检验对数据进行统计分析。结果门诊治疗组男性56例,女性86例,年龄1~11岁,病程2~6d;住院治疗组男性66例,女性99例,年龄0.5~9岁,病程3~8d;住院与门诊治疗组疾病严重程度差异无统计学意义(χ^(2)=3.919,P=0.048)。门诊结合ATSP护理模式治疗的142例患儿,治愈率86.6%,疗程4~12d;住院治疗患儿,治愈率87.2%,两组并发症发生率分别为0.7%和1.2%。患儿家属对治疗过程满意度方面,门诊和住院治疗组分别为99.3%和96.4%,差异无统计学意义(χ^(2)=0.034,P>0.01).结论ATSP护理模式辅助治疗儿童下呼吸道感染,可获得理想的治疗效果,同时有利于降低治疗费用和提高治疗满意度。

Objective To discuss the effect of nurse-led home care model in the treatment of children′s lower respiratory tract infection.Methods A total of 307 cases of lower respiratory tract infection diagnosed in a children′s hospital in Hebei Province from January 2020 to December 2020 were selected,and the gender,age,mental diet status,imaging examination,laboratory examination and other indicators of outpatients and inpatients of children were analyzed.The ATSP nursing model includes assessment,nutrition and mental state;temperature management,dynamically grasping the temperature of children;identification and prevention of febrile seizure;psychological counseling,etcetera.These data were statistically analyzed by using independent sample t-test,χ^(2) test or Fisher′s exact test.Results In the outpatient treatment group,there were 56 males and 86 females,aged 1-11 years,and the course of disease was 2-6 days.In the inpatient treatment group,there were 66 males and 99 females,aged 0.5-9 years,and the course of disease was 3-8 days.The difference in disease severity between the inpatient and outpatient treatment groups was not statistically significant(χ^(2)=3.919,P=0.048).Among 142 children treated in outpatient department combined with ATSP nursing mode,the cure rate was 86.6%,and the treatment duration was 4-12 days.In the children treated in inpatient department,the cure rate was 87.2%,and the incidences of complications in the two groups were 0.7%and 1.2%,respectively.The results of the questionnaire showed that the satisfaction of the family members of outpatient and inpatient patients was 99.3%and 96.4%,respectively,with no statistically significant difference(χ^(2)=0.034,P>0.01).Conclusion ATSP nursing model can help to treat lower respiratory tract infection in children,which can obtain ideal therapeutic effect,reduce treatment cost and improve treatment satisfaction.
