单极射频联合超脉冲CO_(2)激光经结膜入路下睑袋整形术在眼周年轻化治疗中的应用

Application of Unipolar Radiofrequency Combined with Superpulse CO_(2) Laser Via Conjunctival Approach in the Treatment of Periocular Rejuvenation

目的:探究单极射频联合超脉冲CO_(2)激光经结膜入路下睑袋整形术在轻中度眼周皮肤松弛及皱纹治疗中的应用效果。方法:选取2017年6月-2022年6月在笔者医院就诊的轻中度眼周皮肤松弛及皱纹就医者182例,随机分为观察组(n=92)和对照组(n=90),对照组给予超脉冲CO_(2)激光经结膜入路下睑袋整形术治疗,观察组在此基础上联合单极射频治疗,对比两组治疗后美学效果、不良反应,以及手术前后症状积分、Fitzpatrick评分及医患治疗满意度。结果:术后1个月,观察组睑袋减轻、眼周皱纹改善、皮肤色泽增强有效率分别为78.26%、81.52%、85.87%,高于对照组的64.44%、67.78%、73.33%(P<0.05);术后6个月,观察组睑袋减轻、眼周皱纹改善、皮肤色泽增强有效率分别为95.65%、96.74%、94.57%,高于对照组的76.67%、81.11%、83.33%(P<0.05);术后6个月,观察组肤色晦暗、色素沉着、眼睑凹陷及皮肤质地各项症状积分均小于对照组(P<0.05);术后1个月及术后6个月观察组Fitzpatrick评分均低于对照组(P<0.05);医生及就医者满意度分别为97.83%、98.91%均高于对照组的88.89%、92.22%(P<0.05);两组并发症对比差异无统计学意义(P>0.05)。结论:单极射频联合超脉冲CO_(2)激光经结膜入路下睑袋整形术可有效改善就医者眼周皮肤松弛与皱纹,促进眼周年轻化。

Objective To explore the application effect of unipolar radiofrequency combined with ultrapulse CO_(2) laser lower eyelid blepharoplasty through conjunctival approach in patients with mild to moderate periocular skin laxity and wrinkles.Methods A total of 182 patients with mild to moderate periocular skin laxity and wrinkles in the hospital were enrolled and randomly divided into observation group(n=92)and control group(n=90)between June 2017 and June 2022.The control group was given ultrapulse CO2 laser lower eyelid blepharoplasty through conjunctival approach,while observation group was additionally treated with unipolar radiofrequency.The aesthetic effect and adverse reactions after treatment,scores of symptoms and Fitzpatrick before and after surgery,and doctor-patient treatment satisfaction were compared between the two groups.Results At 1 month after surgery,response rates of eyelid bag remission,periocular wrinkle improvement and skin color enhancement in observation group were 78.26%,81.52%and 85.87%,higher than 64.44%,67.78%and 73.33%in control group(P<0.05).At 3 months after surgery,response rates of the three indexes in observation group were 95.65%,96.74%and 94.57%,higher than 76.67%,81.11%and 83.33%in control group(P<0.05).At 6 months after surgery,scores of symptoms(dark complexion,pigmentation,eyelid depression,skin texture)in observation group were lower than those in control group(P<0.05).At 1 month and 6 months after surgery,fi tzpatrick scores in observation group were lower than those in control group(P<0.05).The satisfaction rates of doctors and patients in observation group were 97.83% and 98.91%,higher than 88.89% and 92.22%in control group(P<0.05).There was no signifi cant diff erence in complications between the two groups(P>0.05).Conclusion Unipolar radiofrequency combined with ultrapulse CO_(2) laser lower eyelid blepharoplasty through conjunctival approach can eff ectively improve periocular skin laxity and wrinkles,and promote periocular rejuvenation in patients.
