自体角膜缘干细胞移植治疗翼状胬肉的美学效果及对视力和屈光度的影响

Aesthetic Effect of Limbal Stem Cell Autologous Transplantation in Treatment of Pterygium and Its Infl uence on Visual Acuity and Diopter

目的:探究自体角膜缘干细胞移植治疗翼状胬肉的美学效果及对视力和屈光度的影响。方法:回顾性分析2019年3月-2021年3月就诊于笔者医院的82例翼状胬肉患者资料,根据治疗方式将患者分为对照组(n=40)和联合组(n=42)。对照组接受翼状胬肉切除术,联合组接受翼状胬肉切除术联合自体角膜缘干细胞移植。比较两组患者术后康复情况,手术前后视力与屈光度、并发症发生率及美学效果。结果:联合组患者移植片愈合时间、角膜上皮修复时间及眼部刺激症消失时间均短于对照组(P<0.05);联合组患者总有效率(95.24%)高于对照组(77.50%)(P<0.05);两组患者手术后视力与角膜散光度、角膜水平曲度、角膜垂直曲度等角膜屈光度较手术前显著改善(P<0.05),手术后联合组视力与角膜水平曲度、角膜垂直曲度显著高于对照组,角膜散光度显著低于对照组(P<0.05);联合组患者并发症发生率与复发率(7.14%与4.76%)低于对照组(25.00%与22.50%)(P<0.05)。结论:翼状胬肉切除术患者接受自体角膜缘干细胞移植有助于提高美学效果,改善患者视力及屈光度,疗效优异。

Objective To explore the aesthetic effect of limbal stem cell autologous transplantation on pterygium and its infl uence on visual acuity and diopter.Methods The data of 82 patients with pterygium admitted to the author's hospital from March 2019 to March 2021 were retrospectively analyzed.According to the treatment method,the patients were divided into control group(n=40)and combination group(n=40).The control group received pterygium excision,and the combination group received pterygium excision combined with autologous limbal stem cell transplantation.Postoperative rehabilitation,vision and diopter before and after operation,the incidence of complications,and aesthetic eff ect were compared between the groups.Results The transplant healing time,corneal epithelial repair time and eye irritation disappearance time in the combination group were shorter than those in the control group(P<0.05).The total eff ective rate in the combination group(95.24%)was higher than that in the control group(77.50%)(P<0.05).Vision,corneal astigmatism,horizontal corneal curvature,and vertical corneal curvature of the two groups were signifi cantly improved after operation(P<0.05).After operation,vision,horizontal corneal curvature and vertical corneal curvature of the combination group were signifi cantly higher than those of the control group,and corneal astigmatism was signifi cantly lower than that of the control group(P<0.05).The incidence of complications and recurrence rate in the combination group(7.14%and 4.76%)were lower than those in the control group(25.00%and 22.50%)(P<0.05).Conclusion Autologous limbal stem cell transplantation can improve the aesthetic effect,vision and diopter of patients undergoing pterygium excision,with excellent therapeutic eff ect.
