晚期胃癌HER2表达与MSCT多参数影像表现相关性分析

Correlation Analysis of HER2 Expression in Advanced Gastric Cancer and Multiparametric Imaging Performance of MSCT

目的:探讨人表皮生长因子受体-2(HER2)在晚期胃癌患者中的表达与MSCT多参数影像表现的相关性。方法:回顾性分析2017年6月至2021年12月期间62例晚期胃癌患者临床资料及MSCT扫描资料,根据HER2表达情况的免疫组化结果,晚期胃癌患者分为HER2阴性组47例(75.8%)和HER2阳性组15例(24.2%)。先采用卡方检验对临床病历资料(年龄、性别、分化程度、Lauren分型)及CT多参数影像表现数据(肿瘤部位、厚度、大小、强化方式、强化程度、是否淋巴结转移、是否肝转移及是否腹膜转移)进行分析,再将有统计学意义的因素用Logistic回归进行分析,计算优势比(dominance ratio,OR)。结果:明显强化、是否肝转移、是否腹膜转移,两组间差异具有统计学意义(P<0.05)。年龄、性别、分化程度、Lauren分型、肿瘤部位、厚度、大小、强化方式、是否淋巴结转移在两组间差异无统计学意义(P>0.05)。Logistic回归分析显示,明显强化(OR=8.214)、肝脏转移(OR=6.244)与HER2阳性有关(P<0.05)。结论:与HER2阴性胃癌相比,HER2阳性胃癌表现强化程度(明显强化)、肝脏转移,具有一定潜在的临床应用参考价值。

Objective:To investigate the correlation between the expression of human epidermal growth factor receptor-2(human epidermal growth factor receptor-2,HER2)and multi-parametric imaging performance of MSCT in patients with advanced gastric cancer.Methods:The clinical data and MSCT scans of 62 patients with advanced gastric cancer from June 2017 to December 2021 were retrospectively analyzed,and based on the immunohistochemistry results of HER2 expression,patients with advanced gastric cancer were divided into 47 cases(75.8%)in the HER2-negative group and 15 cases(24.2%)in the HER2-positive group.The clinical record data(age,gender,degree of differentiation,Lauren's typing)and CT multiparametric imaging manifestations data(tumor site,thickness,size,mode of enhancement,degree of enhancement,whether lymph node metastasis,whether liver metastasis and whether peritoneal metastasis)were first analyzed using chi-square test,and then the statistically significant factors were analyzed by logistic regression to calculate the dominance ratio(OR).Results:Significant enhancement,whether liver metastasis,and whether peritoneal metastasis were statistically significant(P<0.05)between the two groups.Age,gender,degree of differentiation,Lauren's staging,tumor site,thickness,size,mode of enhancement,and whether lymph node metastasis were not statistically significant between the two groups(P>0.05).Logistic regression analysis showed that significant enhancement(OR=8.214)and liver metastasis(OR=6.244)were associated with HER2 positivity(P<0.05).Conclusion:Compared with HER2-negative gastric cancer,HER2-positive gastric cancer exhibits a degree of enhancement(significant enhancement),liver metastasis,and has some potential reference value for clinical application.
