血清IL-6、PCT、hs-CRP联合检测在儿童疱疹性咽峡炎合并细菌感染中的应用价值

Application value of combined detection of serum IL-6,PCT and hs-CRP in children with herpangina complicated with bacterial infection

目的分析血清白细胞介素-6(IL-6)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)在儿童疱疹性咽峡炎合并细菌感染中的表达情况及诊断价值。方法选取2021年4月至2022年4月收治的28例疱疹性咽峡炎合并细菌感染患儿作为观察组,另选取同期收治的24例单纯疱疹性咽峡炎患儿作为对照组1,同时选取30例健康体检儿童作为对照组2。检测三组的IL-6、PCT、hs-CRP水平,分析IL-6、PCT、hs-CRP单独及联合检测在疱疹性咽峡炎合并细菌感染患儿中的诊断价值。结果观察组的IL-6、PCT、hs-CRP水平高于对照组1和对照组2,差异具有统计学意义(P<0.05)。IL-6+PCT+hs-CRP联合检测对疱疹性咽峡炎合并细菌感染的阳性检出率高于IL-6、hs-CRP单项检测。IL-6+PCT+hs-CRP联合检测的灵敏度高于IL-6和hs-CRP单项检测,差异具有统计学意义(P<0.05)。IL-6+PCT+hs-CRP联合检测的特异度低于PCT和hs-CRP单项检测,差异具有统计学意义(P<0.05)。IL-6+PCT+hs-CRP联合检测的阳性预测值低于PCT单项检测,差异具有统计学意义(P<0.05)。IL-6+PCT+hs-CRP联合检测的阴性预测值高于hs-CRP单项检测,差异具有统计学意义(P<0.05)。结论血清IL-6、PCT、hs-CRP在疱疹性咽峡炎合并细菌感染患儿诊断中具有一定指导意义,联合检测可以提升其诊断灵敏度和阴性预测值。

Objective To analyze the expression and diagnostic value of serum interleukin-6(IL-6),procalcitonin(PCT)and high-sensitivity C-reactive protein(hs-CRP)in children with herpangina complicated with bacterial infection.Methods From April 2021 to April 2022,28 children with herpangina complicated with bacterial infection were selected as the observation group,and 24 children with simplex herpangina were selected as the control group 1.At the same time,30 healthy children were selected as the control group 2.The levels of IL-6,PCT and hs-CRP in the three groups were detected.The diagnostic value of single and combined detection of IL-6,PCT and hs-CRP in children with herpangina complicated with bacterial infection were analyzed.Results The levels of IL-6,PCT and hs-CRP in the observation group were higher than those in the control group 1 and the control group 2,and the differences were statistically significant(P<0.05).The positive detection rate of IL-6+PCT+hs-CRP combined detection for herpangina complicated with bacterial infection was higher than that of IL-6 and hs-CRP single detection.The sensitivity of combined detection of IL-6+PCT+hs-CRP was higher than that of single detection of IL-6 and hs-CRP,and the difference was statistically significant(P<0.05).The specificity of combined detection of IL-6+PCT+hs-CRP was lower than that of single detection of PCT and hs-CRP,and the difference was statistically significant(P<0.05).The positive predictive value of IL-6+PCT+hs-CRP combined detection was lower than that of PCT single detection,and the difference was statistically significant(P<0.05).The negative predictive value of the combined detection of IL-6+PCT+hs-CRP was higher than that of hs-CRP single detection,and the difference was statistically significant(P<0.05).Conclusion Serum IL-6,PCT and hs-CRP have certain guiding significance in the diagnosis of children with herpangina complicated with bacterial infection,and combined detection can improve its diagnostic sensitivity and negative predictive value.
