集束化护理干预结合体位交换护理对支气管肺发育不良早产儿呼吸动力学指标及生长发育情况的影响

Effects of cluster nursing intervention combined with position exchange nursing on respiratory dynamics indexes and growth and development of premature infants with bronchopulmonary dysplasia

目的探究集束化护理干预结合体位交换护理在支气管肺发育不良早产儿中的干预效果及对呼吸动力学指标、生长发育情况的影响。方法选取2020年1月至2021年12月收治的100例支气管肺发育不良早产儿为研究对象,以随机数字表法将其分为对照组与观察组,各50例。对照组采用常规护理干预,观察组在对照组基础上加施集束化护理干预结合体位交换护理。比较两组的呼吸动力学指标、临床指标、生长发育情况及并发症发生情况。结果干预后,观察组的气道阻力(R)低于对照组,胸肺顺应性(TC)高于对照组(P<0.05)。观察组的呼吸机应用时间、吸氧时间、住院时间均短于对照组(P<0.05)。干预后,观察组的头围、身长、体质量大于对照组(P<0.05)。观察组的并发症总发生率为8.00%,低于对照组的24.00%(P<0.05)。结论集束化护理干预结合体位交换护理能够改善肺部发育不良早产儿的呼吸状态,缩短治疗周期,减少并发症发生风险,使其身体能够正常发育,值得推广。

Objective To explore the intervention effect of cluster nursing intervention combined with position exchange nursing in premature infants with bronchopulmonary dysplasia and its influences on respiratory dynamics indexes and growth and development.Methods A total of 100 premature infants with bronchopulmonary dysplasia admitted from January 2020 to December 2021 were selected as the research objects,and the premature infants were divided into control group and observation group by random number table method,with 50 cases in each group.The control group was given routine nursing intervention,and the observation group was given cluster nursing intervention combined with position exchange nursing on the basis of the control group.The respiratory dynamics indexes,clinical indexes,growth and development and complications were compared between the two groups.Results After intervention,the airway resistance(R)of the observation group was lower than that of the control group,and the thoracopulmonary compliance(TC)was higher than that of the control group(P<0.05).The ventilator application time,oxygen inhalation time and hospitalization time of the observation group were shorter than those of the control group(P<0.05).After intervention,the head circumference,body length and body weight of the observation group were higher than those of the control group(P<0.05).The total incidence of complications in the observation group was 8.00%,which was lower than 24.00%in the control group(P<0.05).Conclusion The cluster nursing intervention combined with position exchange nursing can improve the respiratory state of premature infants with pulmonary dysplasia,shorten the treatment cycle,reduce the risk of complications,and enable the normal development of the premature infants body,which is worthy of promotion.
