三黄汤熏洗在嵌顿痔手术前的临床应用效果研究

Clinical application of Sanhuang Decoction fumigation before incarcerated hemorrhoids operation

目的:探讨三黄汤熏洗在嵌顿痔手术前的临床应用效果。方法:选取2020年12月—2021年6月在我院就诊的嵌顿痔患者98例,按照随机数字表法分为对照组和观察组,均49例。对照组予硫酸镁注射液+盐酸利多卡因湿敷;观察组予三黄汤熏洗;比较两组治疗1 d、3 d、7 d的临床疗效、临床症状(中医症候评分、发热情况)、炎性因子水平及术后恢复情况。结果:观察组治疗1 d、3 d、7 d总有效率为20.41%、53.06%、91.84%,均较对照组6.12%、30.61%、75.51%高(P<0.05);观察组治疗后疼痛、便血、肛缘肿胀的中医证候积分均较对照组低(P<0.05);观察组治疗后体温、白细胞计数水平均较对照组低(P<0.05);观察组治疗后肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β、IL-6、C反应蛋白(CRP)水平较对照组低(P<0.05);观察组术后疼痛评分较对照组低,伤口愈合时间较对照组短(P<0.05)。结论:三黄汤可下调嵌顿痔患者炎症水平,改善疼痛、发热、肛缘肿胀等临床症状,提高治疗效果,促进术后快速恢复。

Objective To investigate the clinical application of Sanhuang Decoction fumigation before incarcerated hemorrhoids operation.Methods A total of 98 patients with incarcerated hemorrhoids treated in our hospital from December 2020 to June 2021 were enrolled and divided into control group and observation group according to the random number table method,with 49 cases in each group.The control group was wet packed with magnesium sulfate injection and lidocaine hydrochloride.The observation group was fumigated with Sanhuang decoction.The clinical efficacy,clinical symptoms(TCM symptom score,fever),levels of inflammatory factors and postoperative recovery were compared between two groups after treatment for 1 d,3 d and 7 d.Results The total effective rates of the observation group on 1 d,3 d and 7 d were 20.41%,53.06%and 91.84%,which were higher than those of the control group(6.12%,30.61%and 75.51%)(P<0.05).The TCM syndrome scores of pain,hematochezia and anal swelling in the observation group after treatment were lower than those in the control group(P<0.05).The body temperature and white blood cell count of the observation group were lower than those of the control group after treatment(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6)and C-reactive protein(CRP)in observation group were lower than those in control group(P<0.05).The postoperative pain score of the observation group was lower than that of the control group,and the wound healing time was shorter(P<0.05).Conclusion Sanhuang Decoction can reduce the inflammation level of incarcerated hemorrhoids patients,improve pain,fever,anal swelling and other clinical symptoms,improve the therapeutic effect and promote rapid recovery after surgery.
