多元化健康教育联合情绪引导在ESD消化道早癌患者中的应用

Application of diversified health education combined with emotional guidance in the nursing of patients with early gastrointestinal cancer after endoscopic submucosal dissection

目的探讨多元化健康教育联合情绪引导在内镜黏膜下剥离术(ESD)消化道早癌患者护理中的应用。方法选取2020年2月至2022年2月江苏省淮安市第二人民医院收治的104例ESD消化道早癌患者通过红蓝双色球法随机分为常规组和联合组,每组52例。常规组给予情绪引导进行干预,联合组在常规组基础上加用多元化健康教育干预。比较2组手术治疗情况和并发症发生情况,干预前后心理应激情况和生活质量。结果联合组比常规组手术时间、术后禁食时间、术后住院时间显著更短,术后复发率显著更低(P<0.05)。干预后2组躯体化、强迫、人际关系敏感、抑郁、焦虑、敌对、恐怖、偏执、精神病性、其他评分均显著降低,联合组比常规组显著更低(P<0.05)。干预后2组生理功能、生殖功能、机体疼痛、健康总体评分、活力、社会功能、情感职能、精神健康评分均显著升高,联合组比常规组显著更高(P<0.05)。联合组比常规组并发症发生率显著更低(P<0.05)。结论应用多元化健康教育联合情绪引导对ESD消化道早癌患者进行干预,显著改善患者手术治疗情况、心理应激情况、生活质量,降低并发症发生率。

Objective To explore the application of diversified health education combined with emotional guidance in the nursing of patients with early gastrointestinal cancer after endoscopic submucosal dissection(ESD).Methods From February 2020 to February 2022,104 patients with early gastrointestinal cancer after ESD treated in Huai’an Second People’s Hospital were randomly divided into the conventional group(52 cases)and the combined group(52 cases)by the red and blue two-color ball method.Patients in both groups were managed by emotional guidance,and those in the combined group were additionally given diversified health education intervention.The surgical treatment and complications,psychological stress and quality of life before and after the intervention were compared between the two groups.Results Compared with those of the conventional group,the operation time,postoperative fasting time,postoperative length of stay in the combined group were significantly shorter,and the postoperative recurrence rate was significantly lower(P<0.05).After the intervention,the scores of somatization,compulsion,interpersonal sensitivity,depression,anxiety,hostility,terror,paranoia,psychosis,and other scores in the two groups were significantly reduced,which were significantly lower in the combined group than those of the conventional group(P<0.05).After the intervention,the scores of physiological function,reproductive function,body pain,overall health score,vitality,social function,emotional function,and mental health of the two groups were significantly enhanced,which were significantly higher in the combined group than those of the conventional group(P<0.05).The incidence of complications in the combined group was significantly lower than that of the conventional group(P<0.05).Conclusion The application of diversified health education combined with emotional guidance can significantly improve the surgical treatment outcome,psychological stress,quality of life,and reduce the incidence of complications in patients with early gastrointestinal cancer after ESD.
