摘要
目的:分析子宫肌瘤和子宫腺肌病应用彩色多普勒超声诊断的应用价值。方法:选取2018年6月—2023年6月无锡虹桥医院接收的60例经手术治疗且病理确诊的子宫肌瘤、子宫腺肌病患者,根据患者病理结果分为宫肌瘤组(n=42)、子宫腺肌病组(n=18),分析两组彩色多普勒超声诊断结果、超声特征及子宫动脉血流参数。结果:彩色多普勒超声检出子宫肌瘤38例,准确率90.48%,子宫腺肌病16例,准确率88.89%,两组超声检查准确率比较差异无统计学意义(P> 0.05);两组患者包块直径以及内部回声比较差异不显著(P> 0.05);子宫腺肌症组单发占比高于子宫肌瘤组,体边界不清晰、瘤体囊实性占比低于子宫肌瘤组,差异有统计学意义(P <0.05);子宫腺肌病患者的搏动指数(PI)、血流阻力指数(RI)、收缩期峰值血流速度(Vs)、舒张期峰值血流速度(Vd)均高于子宫肌瘤患者(P <0.05)。结论:经彩色多普勒超声诊断可进一步明确子宫肌瘤、子宫腺肌症间的差异,对临床诊断有一定借鉴意义。
Objective To analyze the value of color Doppler ultrasonography in the diagnosis of uterine fibroids and adenomyosis.Methods 60 patients with pathologically confirmed uterine fibroids and adenomyosis who were admitted to Wuxi Hongqiao Hospital from June 2018 to June 2023 were selected and divided into uterine fibroids group(n=42)and adenomyosis group(n=18)according to the pathological results.The diagnostic results,ultrasonic characteristics and uterine artery blood flow parameters of the two groups were analyzed.Results The accuracy of color Doppler ultrasonography was 90.48%in 38 cases of uterine fibroids and 88.89%in 16 cases of adenomyosis.There was no statistical significance in the accuracy of ultrasonography between the two groups(P>0.05).There were no significant differences in mass diameter and internal echo between the two groups(P>0.05);The proportion of single cases in adenomyosis group was higher than that in uterine fibroids group,the proportion of body boundary was not clear and the proportion of tumor cyst was lower than that of uterine fibroids group,the difference were statistically significant(P<0.05).The pulsatility index(PI),blood flow resistance index(RI),peak systolic blood flow velocity(Vs)and peak diastolic blood flow velocity(Vd)were higher than those in uterine fibroids(P<0.05).Conclusion After the diagnosis of color Doppler ultrasound,the differences between uterine fibroids and adenomyosis can be further clarified,which has some reference significance for clinical diagnosis.
