药物重整用于老年慢性阻塞性肺病患者的效果评价

Evaluation of the Effect of Medication Reconciliation on Elderly Patients with Chronic Obstructive Pulmonary Disease

目的探索临床药师对老年慢性阻塞性肺病患者实施药物重整的工作模式及效果,为基层药师实施药物重整工作提供参考。方法将萍乡市人民医院2021年5月至2022年5月收治的老年慢性阻塞性肺病患者随机分为对照组和试验组。对照组按传统医疗模式治疗,试验组在传统医疗模式上,增加药物重整。比较两组患者药物不良反应/事件(ADR/ADE)发生率,慢性阻塞性肺疾病评估测试(CAT)评分,住院期间药品金额、药占比、总费用、住院天数以及住院患者满意度。结果住院期间试验组与对照组相比,ADR/ADE发生率、药品金额以及药占比均更低(P<0.05),患者满意度更高(P<0.05),而总费用、住院天数以及出院CAT评分均差异无统计学意义(P>0.05)。结论药物重整能进一步降低药品金额、药占比,可保障慢性阻塞性肺疾病患者用药安全,提高患者满意度,是基层临床药师为患者服务的重要模式。

Objective To explore the working mode and effect of medication reconciliation for elderly patients with chronic obstructive pulmonary disease,and to provide reference for grass-roots pharmacists in medication reconciliation.Methods Elderly patients with chronic obstructive pulmonary disease admitted to Pingxiang People's Hospital from May 2021 to May 2022 were randomly divided into a control group and an experimental group.The control group was treated according to the traditional medical mode,while the experimental group was treated with medication reconciliation in addition to the traditional medical mode.The incidence of adverse drug reaction/event(ADR/ADE),chronic obstructive pulmonary disease score(CAT score),drug amount during hospitalization,drug proportion,total cost,length of stay and patient satisfaction were compared between the two groups.Results During the hospitalization period,the incidence of ADR/ADE,drug amount,and drug proportion were lower in the experimental group compared to the control group(P<0.05),and the satisfaction of hospitalized patients was higher(P<0.05).However,there was no significant difference in total cost,length of stay,and discharge CAT score(P>0.05).Conclusion Medication reconciliation can further reduce the amount and proportion of drugs,ensure patient medication safety,and improve patient satisfaction.It is an important model for grassroots clinical pharmacists to serve patients.
