广东省河源市连平县人乳头瘤病毒流行特点及宫颈高级别病变与宫颈癌检出情况分析

Epidemiological characteristics analysis of human papillomavirus and detection of high-grade cervical lesions and cervical cancer in Lianping County,Heyuan City,Guangdong Province

目的基于以普通人群开展宫颈癌的筛查,分析广东省河源市连平县地区女性人乳头瘤病毒(HPV)的流行特点,并结合组织病理诊断评估宫颈高级别病变和宫颈癌检出情况。方法回顾性分析宫颈癌筛查的26043人检测数据、临床资料和病理诊断结果,分析HPV感染情况及宫颈高级别病变的发生比例。对HPV16/18阳性和细胞学阳性者转诊行阴道镜检查及活检。结果在接受筛查的26043例女性中,HPV阳性者共2701例,HPV感染率为10.37%。HPV16、HPV18基因型和其他高危HPV亚型的感染率分别为0.78%、0.41%和8.82%。450例阳性病例进行组织病理活检,组织病理检查率为65.98%。共检出宫颈炎188例,宫颈上皮内瘤变(CIN)1级病变144例,CIN 2级45例,CIN3级50例,宫颈癌23例。在26043例筛查人群中,共检出CIN2级及以上(CIN2+)118例,共检出CIN3+73例,宫颈癌23例,宫颈癌检出率90/10万。结论河源市连平县女性HPV感染率为10.37%,宫颈癌检出率高达90/10万,亟需在当地适龄女性中开展常规宫颈癌筛查,降低人群宫颈癌发病情况。

Objective To based on the screening of cervical cancer in the general population,the epidemiological characteristics of female human papillomavirus(HPV)in Lianping County,Heyuan City,Guangdong Province were analyzed,and the high-grade cervical lesions and cervical cancer detection were evaluated in combination with histopathological diagnosis.Methods The test data,clinical data and pathological diagnosis results of 26043 patients with cervical cancer screening were retrospectively analyzed,and the HPV infection situation and the incidence of high-grade cervical lesions were analyzed.Patients with HPV16/18 positive and cytological results were referred for colposcopy and biopsy.Results Of the 26043 women screened,2701 were HPV-positive,with an HPV infection rate of 10.37%.The infection rates of HPV16,HPV18 genotype and other highrisk HPV subtypes were 0.78%,0.41%and 8.82%,respectively.The histopathological biopsy was performed in 450 positive cases,and the histopathological examination rate was 65.98%.A total of 188 cases of cervicitis,144 cases of cervical intraepithelial neoplasia(CIN)grade 1,45 cases of CIN grade 2,50 cases of CIN3 and 23 cases of cervical cancer were detected.Among the 26043 screening population,118 cases of CIN2 and above(CIN2+),73 cases of CIN3+and 23 cases of cervical cancer were detected,and the detection rate of cervical cancer was 90/100 o00.Conclusion In Lianping County,Heyuan City,the female HPV infection rate is 10.37%,and the detection rate of cervical cancer is as high as 90/100000.It is urgent to carry out routine cervical cancer screening among local women of appropriate age to reduce the incidence of cervical cancer in the population.
