血必净辅助治疗创伤后瘀毒内阻型脓毒症临床研究

Clinical Study on Xuebijing in the Adjuvant Treatment of Post-traumatic Stasis with Internal Resistance of Sepsis and Toxin

目的探讨血必净辅助治疗创伤后瘀毒内阻型脓毒症的临床疗效。方法选取医院急诊重症监护室(ICU)病房收治的创伤后瘀毒内阻型脓毒症患者60例,随机分为观察组和对照组,各30例。两组患者均予西医常规治疗,观察组患者加用血必净治疗。比较两组患者治疗前后白细胞计数(WBC)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、D-二聚体(D-D)、血小板计数(PLT)的变化情况,以及两组患者的住院时间和死亡率。结果治疗前,两组患者血清WBC,PLT,D-D,APTT水平无显著差异(P>0.05);与治疗前比较,对照组患者治疗后的血清WBC,PLT,D-D,APTT水平无显著差异(P>0.05),观察组患者治疗后的血清WBC,PLT,D-D,APTT水平显著降低(P<0.05);观察组患者治疗后的血清WBC,PLT水平显著低于对照组(P<0.05)。与对照组比较,观察组患者住院时间显著缩短(P<0.05),且死亡率也有一定下降。结论血必净辅助治疗创伤后瘀毒内阻型脓毒症,可有效抑制创伤性脓毒症患者的炎性反应,调节其凝血功能,缩短住院时间及降低死亡率。

Objective To investigate the clinical efficacy of Xuebijing in the adjuvant treatment of post-traumatic sepsis with stasis and toxin internal resistance.Methods Sixty patients with post-traumatic stasis with internal obstruction of sepsis and toxin admitted to the emergency intensive care unit(ICU)of the hospital were randomly divided into the observation group and the control group,with 30 cases in each group.Both groups received routine western medicine treatment,while the observation group received additional treatment with Xuebijing.The changes in white blood cell count(WBC),activated partial thromboplastin time(APTT),thrombin time(TT),D-dimer(D-D),platelet count(PLT),the hospital stay and mortality rate before and after treatment between two groups were compared.Results Before treatment,there was no significant difference in serum WBC,PLT,D-D,APTT levels between the two groups(P>0.05).Compared with those before treatment,there was no significant difference in the serum WBC,PLT,D-D,APTT levels in the control group after treatment(P>0.05),while the serum WBC,PLT,D-D,APTT levels in the observation group after treatment significantly reduced(P<0.05).The serum WBC and PLT levels in the observation group after treatment were significantly lower than those in the control group(P<0.05).Compared with those in the control group,the hospital stay in the observation group significantly shortened(P<0.05),and the mortality rate also decreased to a certain extent.Conclusion Xuebijing can effectively inhibit the inflammatory response,regulate coagulation function,shorten hospitalization time,and reduce mortality in patients with post-traumatic stasis with internal obstruction of sepsis and toxin.
