家庭因素对伴有非自杀性自伤抑郁症青少年治疗的影响

Influence of Family Factors on the Treatment of Adolescents with Non-Suicidal Self-Injury Depression

目的探讨家庭因素对伴有非自杀性自伤(NSSI)抑郁症青少年治疗效果的影响。方法选取2020年6月至2022年6月就诊于福建省福州神经精神病防治院的抑郁症患者93例,按父母是否离异分为试验组(32例,父母离异,包括离异后重组家庭)和对照组(61例,父母未离异)。两组患者均给予抗焦虑药、助睡眠等对症处理,给予舍曲林50~100 mg,每日1次。结果试验组患者的汉密顿抑郁量表(HAMD)评分在入组时及治疗后均显著高于对照组(P<0.05)。试验组患者的汉密顿焦虑量表(HAMA)评分在治疗8,12周显著高于对照组(P<0.05)。治疗后8,12周,对照组的NSSI频率显著高于试验组(P<0.05)。试验组患者有组织损伤的NSSI占比显著高于对照组(P<0.01)。治疗后8周,两组患者的总有效率均明显上升,但差异无统计学意义(P>0.05)。两组患者痊愈率低,但差异无统计学意义(P>0.05)。结论离异与重组家庭的青少年抑郁症状严重,非自杀性自伤程度严重。所有非自杀性自伤的青少年抑郁症患者的治疗效果均欠佳。

Objective To investigate the influence of family factors on the treatment effect of adolescents with non-suicidal self injury(NSSI)depression.Methods Ninety-three patients with depression admitted to the Fuzhou Neuropsychiatric Prevention and Treatment Hospital from June 2020 to June 2022 were selected and divided into the test group(32 cases,parents divorced,including reorganization of family after divorce)and the control group(61 cases,parents not divorced)according to whether their parents were divorced or not.Both groups were given symptomatic treatments such as anxiolytics and sleep aids,and 50-100 mg of sertraline was given once daily.Results The Hamilton Depression Scale(HAMD)scores in the test group were significantly higher than those in the control group at the time of enrollment and after treatment(P<0.05).The Hamilton Anxiety Scale(HAMA)score in the test group was significantly higher than that in the control group after 8 and 12 weeks of treatment(P<0.05).The frequency of NSSI was significantly higher in the control group at 8 and 12 weeks after treatment(P<0.05).The percentage of NSSIs that coaxed tissue damage in the test group was significantly higher than that in the control group(P<0.01).At 8 weeks after treatment,the total effective rate in both groups increased significantly,but the difference was not statistically significant(P>0.05).The recovery rate was low in both groups,but the difference was not statistically significant(P>0.05).Conclusion Adolescents from divorced and reorganized families had severe depressive symptoms and severe levels of non-suicidal self-injury.All non-suicidal self-injury adolescents with depression had suboptimal treatment outcomes.
