胆管癌手术患者预后影响因素分析

Analysis of prognostic influencing factor in postoperative patients with cholangiocarcinoma

目的分析胆管癌手术患者预后的影响因素。方法选取100例胆管癌手术患者,均进行为期3年的随访,将生存时间≤3年的40例患者作为观察组,将生存时间﹥3年的60例患者作为对照组。分析胆管癌患者生存时间≤3年的危险因素。结果两组患者浸润深度、肿瘤大小、分化程度、淋巴结转移情况、TNM分期、切缘情况比较,差异均有统计学意义(P﹤0.01)。全层浸润、肿瘤大小≥3 cm、分化程度低、淋巴结转移、TNM分期高、切缘阳性均是胆管癌患者生存时间≤3年的独立危险因素(P﹤0.01)。结论全层浸润、肿瘤大小≥3 cm、分化程度低、淋巴结转移、TNM分期高、切缘阳性均是胆管癌患者生存时间≤3年的危险因素,应当引起临床重视,及早给予针对性治疗,最大限度地延长患者生存时间。

Objective To analyze the influencing factors of prognosis in patients with cholangiocarcinoma undergoing surgery.Method A total of 100 surgically treated patients with cholangiocarcinoma were selected.All patients were followed up for 3 years and divided into observation group(n=40,survival time≤3 years)and control group(n=60,survival time>3 years)according to the survival time.The risk factors affecting the survival time of patients with cholangiocarcinoma≤3 years were analyzed.Result There were significant differences for the invasion thickness,tumor size,degree of differentiation,lymph node metastasis,TNM stage,and resection margin(P<0.01);full-thickness invasion,tumor size≥3 cm,low degree of differentiation,lymph node metastasis,high TNM stage,and positive resection margin were risk factors for the survival time of patients with cholangiocarcinoma≤3 years(P<0.01).Conclusion Full-thickness invasion,tumor size≥3 cm,low degree of differentiation,lymph node metastasis,high TNM stage,and positive resection margin are risk factors for the survival time of patients with cholangiocarcinoma≤3 years.Clinical attention should be paid and targeted treatment should be given early to maximize the survival time of patients.
