腹腔热灌注化疗治疗胃癌患者的临床疗效及其与热休克蛋白70、热休克蛋白90、糖类抗原125水平的相关性

Clinical efficacy of hyperthermic intraperitoneal chemotherapy in gastric cancer patients and its correlation with heat shock protein 70,heat shock protein 90 and carbohydrate antigen 125

目的探讨腹腔热灌注化疗(HIPEC)治疗胃癌患者的临床疗效及其与热休克蛋白70(HSP70)、热休克蛋白90(HSP90)、糖类抗原125(CA125)水平的相关性。方法根据术后治疗方法的不同将74例胃癌患者分为对照组和观察组,每组37例,对照组患者进行常规化疗,观察组患者在对照组的基础上进行HIPEC治疗。比较两组患者的临床疗效及HSP90、HSP70、CA125水平,采用Pearson相关分析法分析疗效与HSP70、HSP90、CA125水平的相关性。结果观察组患者的总有效率高于对照组,差异有统计学意义(P﹤0.05)。治疗后,两组患者HSP90、HSP70、CA125水平均低于本组治疗前,观察组患者HSP90、HSP70、CA125水平均低于对照组,差异均有统计学意义(P﹤0.05)。HIPEC治疗胃癌患者的疗效与HSP70、HSP90、CA125水平均呈负相关(P﹤0.05),疗效越好,HSP70、HSP90、CA125水平越低。结论HIPEC治疗胃癌患者可提高临床疗效,降低HSP70、HSP90、CA125水平,且疗效与HSP70、HSP90、CA125水平均呈负相关,疗效越好,HSP70、HSP90、CA125水平越低。HSP70、HSP90、CA125可作为HIPEC治疗胃癌疗效的有效预测指标。

Objective To analyze the clinical efficacy of hyperthermic intraperitoneal chemotherapy(HIPEC)in gastric cancer patients and its correlation with heat shock protein(HSP)70,HSP90 and carbohydrate antigen 125(CA125).Method A total of 74 patients with gastric cancer were divided into the control group(n=37,treated with conventional chemotherapy)and the observation group(n=37,treated with HIPEC on the basis of the control group)according to different postoperative treatment methods.The clinical efficacy and levels of HSP70,HSP90 and CA125 were compared between the two group,and the correlation of clinical efficacy with levels of HSP70,HSP90 and CA125 were analyzed by Pearson correlation analysis.Result The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of HSP90,HSP70 and CA125 in both groups were lower than those before treatment,with lower levels in the observation group than those in the control group,and the differences were statistically significant(P<0.05).The clinical efficacy of HIPEC in treating gastric cancer patients was negatively correlated with the expression of HSP70,HSP90 and CA125(P<0.05),the better the clinical efficacy of HIPEC was,the lower the expression of HSP70,HSP90 and CA125.Conclusion HIPEC in the treatment of gastric cancer can improve clinical efficacy,reduce levels of HSP70,HSP90 and CA125,and the clinical efficacy is negatively correlated with the expression of HSP70,HSP90 and CA125.The better the clinical efficacy is,the lower the expression of HSP70,HSP90 and CA125.The expression of HSP70,HSP90 and CA125 may be effective indicators of the clinical efficacy of HIPEC in the treatment of gastric cancer.
