家庭康复指导联合信息和情感支持对急性髓系白血病造血干细胞移植患者的影响

Effect of Family Rehabilitation Guidance Combined with Informational and Emotional Support in Patients with Acute Myeloid Leukemia Undergoing Hematopoietic Stem Cell Transplantation

目的:探讨家庭康复指导联合信息和情感支持对急性髓系白血病(AML)造血干细胞移植患者家庭功能和生存质量的影响。方法:选取2020年2月1日~2022年7月1日收治的106例AML造血干细胞移植患者为研究对象,采用信封法将患者随机分为对照组和观察组各53例,对照组实施常规出院康复指导,观察组在对照组基础上实施家庭康复指导联合信息和情感支持;比较两组出院时及干预24周后心理状态[采用汉密顿抑郁量表(HAMD)及汉密顿焦虑量表(HAMA)]、家庭功能[采用家庭亲密度与适应性量表(FACESII-CV)]、生存质量[采用癌症患者生存质量测定表(QLQ-C30)]及预后情况[包括感染率、复发率及移植物抗宿主病(GVHD)发生率]。结果:干预24周后,两组HAMD、HAMA评分均低于出院时(P<0.05),且观察组低于对照组(P<0.01);干预24周后,两组FACESII-CV、QLQ-C30评分均高于出院时(P<0.05),且观察组低于对照组(P<0.01);观察组感染率、复发率及GVHD发生率均低于对照组(P<0.05)。结论:对AML造血干细胞移植出院患者,进行家庭康复指导联合信息和情感支持干预,能改善其心理状态、家庭功能,提高生存质量,改善预后。

Objective:To explore the effect of family rehabilitation guidance combined with informational and emotional support on fami-ly function and quality of life in patients with acute myeloid leukemia(AML)undergoing hematopoietic stem cell transplantation.Methods:A total of 106 patients with AML who underwent hematopoietic stem cell transplantation from February 1,2020 to July 1,2022 were select-ed,and randomly divided into the control group and the observation group by the envelope method,with 53 patients in each group.The con-trol group was given routine rehabilitation guidance after discharge,while the observation group was given family rehabilitation guidance combined with informational and emotional support.The two groups were compared in terms of mental state by Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA),family function by Family Adaptability and Cohesion Scale,Second edition,Chinese ver-sion(FACISI-CV),quality of life by Quality of Life Questionnaire-Core 30(QLQ-C30)and the prognosis including infection rate,recur-rence rate and the incidence of graft-versus-host disease(GVHD).Results:After intervention for 24 weeks,the HAMD scores and HAMA scores were lower in both groups than those at discharge(P<0.05),and those in the observation group were significantly lower than those in the control group(P<0.01).After intervention for 24 weeks,the FACESI-CV scores and QLQ-C30 scores were significantly higher than those at discharge(P<0.05),and those in the observation group were lower in the control group(P<0.01).The infection rate,recurrence rate,and the incidence of GVHD in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Family rehabilitation guidance combined with informational and emotional support can significantly improve mental state,family function,quality of life,and the prognosis of patients with AML after hematopoietic stem cell transplantation and discharge from the hospital.
