自行清洁间歇导尿管理在子宫内膜癌根治术后患者中的应用效果分析

Application Effect Analysis of Self-cleaning Intermittent Catheterization in Patients with Endometrial Carcinoma afterRadical Surgery

目的:研究自行清洁间歇导尿管理对子宫内膜癌根治术后患者尿路感染风险及膀胱功能恢复的影响。方法:选取2021年2月—2023年2月南阳医学高等专科学校第一附属医院收治的103例子宫内膜癌根治术后患者为研究对象,采用随机数表法将其分为对照组(n=51)和研究组(n=52)。给予对照组患者术后常规留置导尿管理,给予研究组患者自行清洁间歇导尿管理。结果:术后,研究组患者自主排尿恢复时间为(2.41±0.52)d、残余尿量为(75.44±18.63)mL,均低于对照组,差异有统计学意义(t值分别为14.0122和10.1454,P值均<0.05);研究组患者中膀胱功能恢复良好者46例,良好率为88.46%,高于对照组患者(64.71%),差异有统计学意义(χ2=8.1203,P<0.05);研究组患者舒适程度情况优于对照组,差异有统计学意义(Z=5.1181,P<0.05);研究组患者尿路感染发生率为3.85%、尿管拔除后尿潴留发生率为1.92%,均低于对照组,差异有统计学意义(χ2值分别为5.8139和4.5120,P值均<0.05)。结论:自行清洁间歇导尿管理对子宫内膜癌根治术患者效果显著,能够有效促进患者膀胱功能恢复,降低患者尿路感染发生率和尿管拔除后尿潴留发生率,舒适程度较高,具有临床推广价值。

OBJECTIVE To study the effects of self-cleaning intermittent catheterization on urinary tract infection risk and blad⁃der function recovery in patients with endometrial cancer after radical surgery.METHODS 103 patients with endometrial carcinoma treated in the First Affiliated Hospital of Nanyang Medical College from February 2021 to February 2023 were selected as the study objects,and were divided into control group(n=51)and study group(n=52)by random number table method.The control group was given routine indwelling catheterization management,while the study group was given self-cleaning intermittent catheterization man⁃agement.RESULTS After surgery,the recovery time of spontaneous urination in the study group was(2.41±0.52)d,and the residual urine volume was(75.44±18.63)mL,which were lower than those in the control group,and the differences were statistically signifi⁃cant(t=14.0122,10.1454,P<0.05).In the study group,46 patients had good bladder function,the good rate was 88.46%,which was higher than that of the control group(64.71%),and the difference was statistically significant(χ2=8.1203,P<0.05).The com⁃fort level of the study group was better than that of the control group,and the difference was statistically significant(Z=5.1181,P<0.05).The incidence of urinary tract infection in the study group was 3.85%and the incidence of urinary retention after catheter re⁃moval was 1.92%,both of which were significantly lower than those in the control group(χ2=5.8139,4.5120,P<0.05).CONCLU⁃SION Self-cleaning intermittent catheterization has a significant effect on patients undergoing radical hysterectomy for endometrial cancer,which can effectively promote the recovery of bladder function,reduce the incidence of urinary tract infection and urinary re⁃tention after catheter removal,and has a high degree of comfort and clinical promotion value.
