孕期营养干预及分娩期自由体位对降低剖宫产率的作用探究

摘要 目的 研究孕期营养干预及分娩期自由体位对降低剖宫产率的作用。方法 选取2022年1-12月在本院分娩的108例孕妇,根据双色球法将其随机分为对照组和观察组,每组54例,对照组实施常规孕期保健和卧位分娩,观察组加强孕期营养干预并选择自由体位分娩。将两组的妊娠晚期相关指标、产程与产后出血量、分娩期疼痛、分娩方式、围产儿结局进行比对。结果 观察组的妊娠晚期血红蛋白水平高于对照组,体重增长值、产后出血量均少于对照组,产程时间短于对照组,差异具有统计学意义(均P<0.05);观察组的分娩期疼痛程度轻于对照组,阴道分娩率(87.04%)高于对照组,剖宫产率(5.56%)、围产儿不良结局发生率(7.41%)低于对照组,均P<0.05。结论 通过孕期营养干预与分娩期自由体位能够控制孕妇孕期体重及血红蛋白水平,加快产程进展,减少产后出血量并减轻分娩期疼痛,还可降低剖宫产率,优化母婴结局。 Objective To study effect of nutrition intervention during pregnancy and free body position during delivery on reducing cesarean section rate.Methods The paper chose 108 pregnant women delivered in our hospital from January 2022 to December 2022,and divided them into control group and observation group randomly according to dual color sphere method,with 54 cases in each group.Control group was treated with routine prenatal nursing and lying position delivery,while observation group with enhanced nutritional intervention during pregnancy and free position delivery.Late pregnancy related indicators,labor process and postpartum bleeding,labor pain,delivery mode and perinatal outcomes were compared between two groups.Results Hemoglobin level in the third trimester of pregnancy in observation group was higher than control group,weight gain and postpartum bleeding was lower than control group,labor duration was shorter than control group,with statistically significant difference(P<0.05).Pain degree during delivery in observation group was lighter than control group,vaginal delivery rate(87.04%)was higher than control group,cesarean section rate(5.56%)and incidence of perinatal adverse outcomes(7.41%)were lower than control group,P<0.05.Conclusion Nutrition intervention during pregnancy and free posture during delivery can control weight and hemoglobin level of pregnant women,accelerate labor progress,reduce postpartum bleeding amount and pain during delivery,reduce cesarean section rate and optimize maternal and infant outcomes.
