鼓室灌注甲泼尼龙挽救突发性耳聋治疗效果观察

Curative Effect Observation of Injecting Methylprednisolone in Tympanic Cavity for Salvage of Sudden Deafness

目的 分析鼓室灌注甲泼尼龙对突发性聋的挽救治疗效果。方法 选取2021年1月-2022年12月在本院接受突发性聋治疗的患者50例作为研究对象。研究中将这50例患者归为观察组,另在接受常规治疗后疗效不好但又未接受其他治疗的患者中随机抽取25例作为对照组。观察组的患者接受为期10d的鼓室灌注甲泼尼龙(intratympanic methylprednisolone perfusion,IMP)治疗,然后通过使用纯音测听的方法分析观察组患者接受治疗前后的平均纯音听阈(pure tone average,PTA)的变化。结果 IMP治疗对突发性聋的治愈率高达34.69%,明显比对照组的治愈率16.04%高出许多。接受了IMP治疗之后,患者的听阈水平得到了明显的改善。分析IMP治疗前后听力下降程度的PTA变化、发病时长对PTA的变化、伴随症状对PTA的变化三方面的结果发现,听力下降程度的PTA值和发病时长对PTA值的变化差异都具有统计学意义(P<0.05),但伴随症状对PTA值的变化差异不具有统计学意义(P>0.05)。结论 IMP的挽救性治疗对突发性聋具有良好的治疗效果,值得在临床上应用及推广。

Objective To analyze salvage and treatment effect of tympanic infusion of methylprednisolone on sudden deafness.Methods The paper chose 50 patients with sudden deafness treatment in our hospital from January 2021 to December 2022 as study subjects,and treated them as observation group,and chose 25 cases with low effct after routine treatment and no other treatments as control group.Observation group was treated with 10 days of intratympanic methylprednisolone perfusion(IMP)and changes in average pure tone auditory threshold(PTA)before and after treatment were analyzed with pure tone audiometry.Results Cure rate of IMP treatment for sudden deafness was as high as 34.69%,significantly higher than 16.04%in control group.After IMP treatment,hearing threshold level of patients was significantly improved.PTA changes analysis in hearing loss degree before and after IMP treatment,effect of duration on PTA and effect of accompanying symptoms on PTA showed,there were statistically significant difference in PTA values of hearing loss degree and effect of duration on PTA values(P<0.05),but effect of accompanying symptoms on PTA values was not statistically significant(P>0.05).Conclusion Salvage therapy of IMP can achieve good curative effect on sudden deafness,which is worthy of clinical application and promotion.
