静脉定位仪联合护士分层管理在Ⅲ~Ⅳ级外周静脉留置针穿刺困难患儿中的运用

Application of venous localizer combined with nurse layered management in children with difficulty in puncturing peripheral venous indwelling needles of grade III-IV

目的探讨静脉定位仪联合护士分层管理在外周静脉穿刺困难患儿留置针穿刺中的应用效果。方法选取2021年4—12月入住荆州市某三级甲等医院儿科一区、二区并采用儿童血管评级工具甄别为Ⅲ~Ⅳ级外周静脉穿刺困难的265例患儿作为研究对象,将儿科二区128例患儿设为观察组,在静脉定位仪辅助下对静脉评估分级定位,并由相应穿刺层级的护士进行留置针穿刺;儿科一区137例患儿设为对照组,由相应穿刺层级的护士采用常规方法进行留置针穿刺。比较两组患儿留置针穿刺一次成功率、平均穿刺次数、穿刺时长及患儿家长对护理的满意程度。结果观察组患儿留置针穿刺一次成功率、患儿家长对护理满意程度高于对照组,差异有统计学意义(P<0.05);观察组患儿平均穿刺次数、平均穿刺时长低于对照组,差异有统计学意义(P<0.05)。结论静脉定位仪联合护士分层管理在Ⅲ~Ⅳ级外周静脉穿刺困难患儿的应用,能有效提升儿科护士留置针穿刺一次成功率,减少穿刺次数,减轻患儿痛苦,提高患儿家长对护理的满意程度。

Objective To explore the application effect of vein locator combined with nurse stratified management in the indwelling needle puncture of children with difficulty in peripheral vein puncture.Methods To select 265 pediatric patients who were admitted to a tertiary A hospital in Jingzhou city from April to December 2021 and were diagnosed with difficulty in peripheral vein puncture using the children's vascular rating tool were selected as the research subjects.128 pediatric patients in the second district were selected as the observation group.With the assistance of a venous locator,the veins were evaluated,graded,and located,and the corresponding puncture level nurses performed indwelling needle puncture;137 cases in pediatric division 1 were used as the control group,and nurses at the corresponding puncture level used conventional methods for indwelling needle puncture.The one-time success rate,average number of punctures,average puncture duration,and satisfaction of parents with nursing care were compared between the two groups of children with indwelling needle puncture.Results The one-time success rate of indwelling needle puncture in the observation group was higher than that in the control group,and the satisfaction of parents with nursing care was higher,the differences were statistically significant(P<0.05).The average number of punctures,average puncture duration in the observation group were less than those in the control group,the differences were statistically significant(P<0.05).Conclusion The application of venous locator combined with nurse hierarchical management in children with difficulty in peripheral vein puncture at levels III to IV can effectively improve the one-time success rate of pediatric nurses'indwelling needle puncture,reduce the number of punctures,alleviate the pain of children,and improve the satisfaction of parents with nursing care.
