预见性护理对急诊病房高龄老年患者误吸的效果

The effect of predictive nursing on aspiration of advanced age elderly patients in emergency ward

目的研究预见性护理对急诊病房高龄老年患者误吸的预防效果,为减少高龄老年患者误吸提供帮助。方法选取2017年12月—2020年1月在天津康复疗养中心急诊病房接受治疗的高龄老年患者60例为研究对象,根据随机数字表法将其分为对照组和观察组各30例。对照组患者行常规护理,包括实施饮食指导,确保气道畅通等;观察组患者在常规护理的基础上,分别对低危、中危以及高危患者实施不同预见性护理措施。比较两组误吸、返流、窒息、吸入性肺炎发生率及护理满意率。结果观察组误吸、返流、窒息及吸入性肺炎发生率低于对照组,差异均有统计学意义(P<0.05)。观察组护理总满意率为96.67%,高于对照组的73.33%,差异有统计学意义(χ^(2)=6.405,P<0.05)。结论对急诊病房高龄老年患者实施预见性护理,可以减少患者误吸、返流、窒息及吸入性肺炎等临床情况的发生,同时能提高患者满意率,值得临床推广应用。

Objective To study the prevention effect of predictive nursing on aspiration of advanced age elderly patients in emergency ward,and to provide assistance to reduce aspiration of advanced age elderly patients.Methods Sixty advanced age eldrly patients receiving treatment in the emergency ward of Tianjin Rehabilitation and Sanatorium Center from December 2017 to January 2020 were selected as the study objects and divided into the control group and the observation group,with 30 cases in each group according to the random number table method.The patients in the control group received routine nursing including implementing diet guidance,and ensuring smooth airway;on the basis of routime nursing,the patients in the observation group implemented different predictive nursing measures for low-risk,medium-risk and high-risk patients.The incidences of aspiration,reflux,asphyxia and aspiration pneumonia,and nursing satisfaction rate were compared between the two groups.Results The incidences of aspiration,reflux,asphyxia and aspiration pneumonia in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total nursing satisfaction rate in the observation group was 96.67%,which was lower than that in the control group with 73.33%,and the difference was statistically significant(χ^(2)=6.405,P<0.05).Conclusion The implementation of predictive nursing for advanced age elderly patients in the emergency ward could reduce the occurrence of aspiration,reflux,asphyxia,aspiration pneumonia and other clinical conditions,and improve the satisfaction rate of patients,it was worthy of clinical promotion and application.
